What is the Systems of Care (SOC)?
Family Support
At times children and families can face overwhelming chal-lenges. While services are available in a number of agen-cies, it is often difficult and time consuming to find just the right help and support system. SOC is a group of providers eager to help families find solutions to serious problems.
One Stop Service
SOC is a process where fami-lies can help develop a single plan to access needed services from a variety of agencies. The members of the SOC are com-mitted to working together to provide a comprehensive and coordinated approach to getting families the services and sup-port they need to be successful.
Services can include:
1. Individual and family coun-seling
2. Employment & financial assistance
3. Health care
4. Educational support
5. Substance abuse treatment
6. Parenting information
HOW IT WORKS:
Sometimes parents request
help from the SOC., but many times con-cerned teach-ers, commu-nity mem-bers, or friends will suggest SOC partici-pation. The first step is to learn about SOC and how it can help. Then if you wish to partici-pate, these are the steps to suc-cess:
1. Sign an application form, which allows agencies to share information and work together.
2. A SOC representative will visit with you to get to know your family and needs.
3. The Family Support Team will then meet to develop a family service plan. Family members are welcome to attend this meeting.
4. The SOC representative will monitor both the promised ser-vices and progress of the fam-ily. Parents will be regularly consulted and informed about progress.
5. When the plan is complete, an evaluation of the outcomes will be shared with the family and SOC.
