Standard Contract for Services datedBetween the Cityof Port Orange, Florida and Extensys, Inc.

THIS Task Authorization is entered into by and between the City of Port Orange, Florida, achartered municipal corporation with its principal place of business at 1000 City CenterCircle, Port Orange,Florida 32129 (the "City") and Extensys, Inc, a Florida corporation with its principal place of business at253 Pine Avenue North, Building B, Oldsmar, Florida 34677 ("Contractor"), and hereinafter collectivelyreferred to as the "Parties," and is to that certain Standard Contract for Services dated , andany amendments thereto, hereinafter collectively referred to as the "Contract." The Parties, in exchange forthe mutual covenants contained herein and in the Contract, agree as follows:

1. This Task Authorization expressly modifies the Contract and in the event of a conflict, the termsand conditions of thisTask Authorization shall prevail.

2. In addition to all other terms and conditions contained in the Contract, Contractor shall provideservices relating to Extensys, Inc. Proposal dated February 26, 2016 as more particularlydescribed in the Scope of Services attached hereto and incorporated herein as Task AuthorizationExhibit "A."

3. Contractor shall complete the services to be provided herein within 365 days of the date of writtennotice by the City to the Contractor.

4. In return for the services identified above, the City agrees to compensate Contractor at the pricesset forth in Exhibit "A" attached hereto and made a part hereoffor all purposes, subject to a limit upto but not to exceed $249,470.00. All payments shall be governed by the Local GovernmentPrompt PaymentAct as set forth in Sections 218.70 through 218.79, Florida Statutes, as amended.

5. This Task Authorization may be executed in one or more counterparts, each of which shall bedeemed to be an original but all of which together shall constitute one and the same instrument.The delivery by facsimile or e-mail ofan executed copy of this Task Authorization shall be deemedvalid as if an original signature was delivered. No contract shall be formed between the Contractorand the City until the City signs thisTask Authorization.

EXECUTIVE SUMMARY:

The City of Port Orange has engaged Extensys, Inc. (Extensys) to implement a full VDI solution leveraging vSphereHorizon view, Cisco UCScomputing infrastructure, and Netapp Storage. Extensys will provide for completeinstallation, configuration, and knowledge transfer of this solution. Installation and configuration limited toequipment and software procured through Extensys for this project.
