Severe permanent harm = 17

Death = 87

XXXRecent study reveals up to 440,000 deaths occur annually in hospitals that are preventable; much more harmData from all Quantros database and ShareSuite through 11/1/2013Only a sampling of the harm and death that we know is occurring and that we are working toward preventing. We want to knock out this death and harm!4Why We Do What We Do!

I always like to keep in mind the real reason we do this work. To take care of those we love and serve!It particularly hits home how important this work is when we think about our families.All the way from the 103 grandmother, a mother with chronic diseases, hospitalizations, now in assisted living care to the new generation.And for the Centers newest addition, Will Schelp, Gingers bundle of joy!We thank you for all that you have done and will do into the future to keep our patients and families safe from harm!Introduce Jeanne to share her perspective on this important work and the results of the work youve done over the past two years!5

Good afternoon and welcome.Some of you may know that I am going to become a grandmother for the first time next month.So I have babies on my mind and it struck me one day, think of how carefully we protect our children.As parents our children count on us to provide a reliable safe and secure environment that nurtures their health.The very same things our patients expect from us. Looking at these beautiful little souls, makes me feel so good about working with you to keep our hospitals safe so none of these little ones end up some day with the consequences of something we could have prevented during a hospitalization in their life.

6Harm Across the Board

Some of you did a harm across the board template, this exercise can really make the strengths and areas of opportunity pop for you. It is also a great way to demonstrate your hard work to the leaders of your hospital. Safety begins in the board room and you have to get the message there. 7CUSP CAUTI - 30 Prevented74% decrease for a savings of $22,500
