There is a very valuable lesson for us in their approach that we can utilize today. You have the opportunity today to author your narrative in your community. Most chiropractors are aware of their office’s image and

presence in their community. If you practice in a smaller town like Danville, it is easy to measure your reputation. I first opened my practice in North Garden, Virginia, population 900. In this small town, I found it rather easy to determine how effective I was in marketing my message. However, if you live in the midst of millions of other people in northern Virginia, it can be more difficult to let your community, not to mention your city, know who you are

and what you do. It is the blessing and curse of this profession that no two chiropractors do exactly the same thing in their respective practices. While one of us may never adjust below the atlas, another may focus on rehabilitation of knees. would contend that it is your responsibility to effectively

communicate who you are and what you do on a daily basis to your patients and to your community. You cannot just depend on the VCA or other leaders to promote Chiropractic in your area. There are several primary methods of getting your narrative into your community: your patients, your website, and your print material. I intentionally left out advertising because I am talking about long-term, reliable resources that will have an impact in your area. Most forms of advertising limit space or time, leaving you unable to fully convey who you are and what you do. While advertising can be effective at attracting new patients, it does not keep patients in your office. It’s who you are and what you do that does it! Y o u r website and print material are your voice when you cannot be present. They tell a prospective patient who you are as a person and a Chiropractor. They must be an accurate reflection of you, your office, and what you do. The simplest way of doing this is to buy a template website where some of your personal information is imported and you are able to select pre-written text that may or may not reflect your particular approach. Similarly, finding pre-written printed material that covers topics important to you goes back over one hundred years in this profession, but does a poor job telling YOUR story. With this “one size fits all” method, you’re forced to settle for whatever happens to be available. A better, more effective way is to create customized material in both digital and print media that is a true reflection of your practice and style. This helps eliminate confusion or conflicting ideas about what you do and how you do it. It also promotes the most identifiable part of your practice – YOU! Marketing research proves that the personal connection communicated through story or experience carries far more weight in the decision making process than sterile research or letters after your name. So take time to craft your message and let Virginia know who you are! Become the author of your own narrative.

On August 22, 2013, in a first of its kind meeting, coding and reimbursement executives from the VCA

and WellPoint/Anthem met to discuss several issues of importance to Virginia chiropractors. Dr. Bibhu Misra, VCA Vice President and Insurance Chairman stated, “clearly the right experts were involved so no stones were left unturned around established and nationally accepted reimbursement protocols and standards. The meeting was hopeful and the VCA will learn the proper code use and value on Dry Needling from Anthem within two weeks.” Several other outcomes of the meeting place the VCA in a better position to bring about changes that Virginia doctors need and deserve. • Connections have been made at the top for policy

change. VCA is now linked to the national Medical Director for all WellPoint/Anthem plans in 14 states. A conference call took place with him in May 2013 and regular contact continues.

• The Regional Vice President and Medical Director for Anthem is very responsive to VCA about membership concerns.

• The VCA now has higher level contacts to assist us on member concerns with network and claim processing issues.

• The VCA presented to Anthem several state laws that we believe place them at risk for potential unfair practices. • Ethical and fair carrier business practices. • Unfair claim practices. • Bone and joint diagnostic

procedure law.• Reimbursement law.

• Anthem now considers dry needling by a doctor of chiropractic more complex than they originally thought. Dr. Misra presented compelling clinical evidence and provider risk/complexity factors that convinced Anthem to re-evaluate coding for dry needling.

• Anthem staff admitted and VCA objected to claim practices that ignore the full evaluation of claims for unlisted procedures. The added burden placed on DCs to provide a procedure report, which Anthem stated they do not read or use in the claim evaluation for CPT #97139, is viewed as a serious unfair claim practice by the VCA.

• VCA provided several available coding options that would allow improvement in reimbursement.

• A solution around CMT and E/M codes with modifier -25 was reached. • Dr. Misra presented examples of why exams and

treatment would be performed on the same visit for an established patient.

• Anthem agreed that in these cases there was no conflict in the contract language for in-network providers. (If a modifier-25 is not specifically prohibited in a particular section of the contract, it could be used as the re-exam and would be viewed as a separate and distinct procedure from the CMT. Anthem agreed it would be appropriate to bill a modifier-25 on the E/M code if therapy and/or CMT was also provided on this visit.)

• The meeting helped place the chiropractic community and VCA members in a more favorable light; similar

strategies can be used with other large insurers in the state.

In summary, this is a very important step in building relationships with major insurance companies. Honest, straightforward and frank discussions of issues on both sides will go a long way in making things better for chiropractic in Virginia. Another meeting with Anthem is being planned for November to present chiropractic cost-effectiveness research and an Evidence Based Model for Chiropractic Prevention and Wellness. Stay tuned for further developments and what we anticipate will be favorable results for Virginia doctors. The VCA has informed Anthem that we will not go away and that there’s a Plan B in place for regulatory intervention if an acceptable outcome is not forthcoming on Dry Needling.

How Excess Body Weight Promotes Pain

When I was first in chiropractic college, the general belief at the time within chiropractic, physical therapy and medicine was that being overweight would mechanically

strain the low back and cause pain. Subsequent studies demonstrated that there was not a direct correlation between being overweight/obese and back pain. In recent years, studies have demonstrated that some people who are overweight suffer from diabetes or a chronic inflammatory state that precedes diabetes known as

the metabolic syndrome. In other words, some overweight individuals are chronically inflamed, while others are not. With this in mind, researchers have further identified that being overweight/obese contributes to chronic pain when these individuals are chronically inflamed. Table 1 on left outlines the recently studied correlations (BMI, metabolic syndrome, diabetes and pain expression). Body mass index (BMI) Overweight and obese individuals are more likely to suffer from tension-type or migraine headache, fibromyalgia, abdominal pain, and chronic widespread pain Studies have implicated an elevated by BMI as a promoter of low back pain Obese subjects with hsCRP levels above 3 mg/dL are more likely to report low back pain, compared to obese subjects with normal levels

Metabolic syndrome Local and widespread musculoskeletal pains are more common in patients with the metabolic syndrome Prevalence of neck pain is higher in patients with metabolic syndrome Shoulder pain is associated with the metabolic syndrome Achilles, patella, and elbow tendinopathy are associated with the metabolic syndrome Risk of lumbar disc herniation is increased by the metabolic syndrome Low back and radiating pain is associated with elevated serum lipids and cardiovascular disease risk factors Osteoarthritis is promoted by the metabolic syndrome

Type 2 diabetes Patients with type 2 diabetes have reduced mobility across all joints tested compared to age/weight matched controls Patients with type 2 diabetes are more likely to develop lumbar stenosis compared to non-diabetics Type 2 diabetes increases the risk of expressing disc herniation in both the cervical and lumbar spines Continued on page 19

Not surprisingly, the appropriate natural intervention for an excess BMI, the metabolic syndrome, and diabetes, involves diet and exercise. In my view, the operational goal should be to reduce chronic inflammation. Nutritional supplements are also useful in this regard. Two recently published papers that outline this nutritional approach are available at their respective websites (2,3).
