Welcome!Fall is upon us, and what better place to be than beautiful Burlington, Vermont! On behalf of the Northeast Telehealth Resource Center Team, I’d like to extend a warm welcome to each of you, and thank you for joining us for the 2014 NETRC Regional Telehealth Conference. We would also like to extend our appreciation to our funders at the Office for the Advancement of Telehealth-HRSA/DHHS, and to our conference sponsors for your support of this year’s event. Our conference theme is: Leveraging Telehealth to Achieve the Triple Aim, and we have brought together an excellent group of speakers to present national, regional and local perspectives around how we can best use technology to increase access to quality care, particularly in respect to rural locations.

As a telehealth resource center, our charge is to provide evidence-based resources and technical assistance to those who are planning, implementing and/or evaluating telehealth programs throughout the

Northeast (and beyond).

Whether you’re just beginning your journey with telehealth, are looking to expand your current program, and/or impact more

global aspects such as policy, we are here and eager to assist.

We hope that you find the information and resources presented here today to be helpful as you build and enhance your telehealth programs. We also encourage you to network with colleagues from around the region, conference exhibitors, and the NETRC team to identify opportunities for collaboration moving forward. DONNA BURKETT, MD is Medical Director of Planned Parenthood of Northern New England and a family physician specializing in reproductive health for women and men. At PPNNE she has worked to improve access to patient-centered reproductive health care for all, including delivery of services to rural populations via telemedicine.

HARRY CLARK is the Senior Videoconferencing Engineer at Fletcher Allen Health Care in Burlington, VT where he fulfills the Telemedicine Coordinator position, providing scheduling and operational oversight for FAHC and University of Vermont College of Medicine videoconferencing, as well as ensuring connectivity to affiliated sites in the North East and across the country. He has also provided technical resource guidance and equipment acquisition expertise over the past fifteen years to FAHC and the UVM/COM, with extensive interaction with videoconference equipment and A/V vendors.

PETER COBB has been director of the VNAs of Vermont, the professional association that represents the Visiting Nurse Associations and non-profit hospice providers in the state, for 34 years. Previously, he was a caseworker for Washington County Mental Health, a reporter for the Barre-Montpelier (VT) Times Argus and a founder of the Vermont Public Interest Research Group.

JOEY MARIE HORTON is Executive Director of the North Country Family Health Center, a federally qualified health center, providing Jefferson, Lewis, St. Lawrence, and Franklin Counties of Northern New York with medical, mental health, dental, and WIC services. Previously, she was Deputy Director of Fort Drum Regional Health Planning Organization, Network Director of North Country Healthcare Providers, a multi-hospital rural health network in Northern New York, and Co-Executive Director of the New York State Coalition for School-Based Health Centers, advocating for NY’s over 200 school-based health centers.

DAVID C. JOHNSON is Telemedicine Program Coordinator for North Country Healthcare Providers, working to expand the reach of telehealth solutions to improve access and quality of health care throughout Northern New York. He is a Certified Telehealth Liaison. Previously, he spent 16 years in the financial services industry as a CERTIFIED FINANCIAL PLANNER™, and held several executive and sales professional positions.

YVONNE LOCKERBY is VP of Health Center Operations at Planned Parenthood of Northern New England in Burlington, VT. Before appointment to her current position, she oversaw the agency’s BlueMail services, which provide contraceptive supplies by mail, the Centralized Lab Management team, Marketing and IT departments, and created the internal department of Centralized Support Services.

DANIELLE LOUDER is Program Manager for Technology Supported Initiatives within Medical Care Development’s Public Health Division, where she guides implementation and growth of the Northeast Telehealth Resource Center, in partnership with the Fletcher Allen Health Care Telemedicine Program at the University of Vermont. She has spent the last fifteen years developing, implementing and evaluating programs focused on increasing access and enhancing quality of care at regional, state-wide and local levels, and played a significant role in the establishment of a network which utilizes telehealth to ensure timely and accurate diagnosis and treatment of stroke throughout the state of Maine.

ROBERT MARTINIANO is Senior Program Manager at the New York Center for Health Workforce Studies, and has extensive background in health workforce research and program management, including 11 years with the New York State Department of Health. He is lead staff at the center on several projects concerning federal shortage designations and New York State workforce assessment, and is an adjunct professor at the Sage Colleges School of Management.

SARAH N. PLETCHER, MD, MA is the Medical Director of the Center for Telehealth at Dartmouth-Hitchcock Medical Center in Lebanon, NH, and Medical Director of Center for Rural Emergency Services and Trauma. She is also Assistant Professor in the Departments of Surgery and Medicine at the Geisel School of Medicine at Dartmouth College. She has been a principal investigator or key personnel on initiatives concerned with telemedicine, innovation, medical education, and outreach program development-particularly for rural communities. Previously, she worked in rural health care in Michigan, and with the National Health Service at King’s College Hospital in London, England.

TERRY RABINOWITZ, DDS, MD is Professor of Psychiatry and Family Medicine at the University of Vermont College of Medicine, and has been Medical Director of the Psychiatry Consultation Service at Fletcher Allen Health Care since 1996. He designed the state’s first telepsychiatry consultation service in 2001. He was appointed Medical Director of telemedicine services at Fletcher Allen/UVM in 2005, and has worked to develop and implement telehealth initiatives with the American Telemedicine Association, the Academy of Psychosomatic Medicine, and the Office for the Advancement of Telehealth, part of the federal Department of Health and Human Services. He earned his MD degree from Case Western Reserve University and did his residency in adult psychiatry at McLean Hospital, Harvard Medical School.

DAVID A. V. REYNOLDS, Dr. PH was Field Representative for Health Care for Senator Patrick Leahy, Deputy Director of Health Reform Policy for Vermont Governor Peter Shumlin, and Senior Health Policy Advisor to Senator Bernie Sanders. He drafted and negotiated key provisions of the Affordable Care Act, focusing on primary care and workforce issues, including expansion and continuing funding of the National Health Service Corps and the Community Health Center program. In 1976, he founded Vermont’s first federally qualified health center network, still providing primary care, dental care, and prescription drugs for 20,000 Vermont residents.

RAE ANN VITALI, MS, CHES is a Health Program Administrator in the Office of Primary Care and Health Systems Management at the New York State Department of Health, where she chairs the Telehealth Executive Workgroup, charged with creating a more favorable policy environment to advance telehealth in New York State. She holds a Master of Science in Health Policy from the University at Albany School of Public Health.

JANICE WATERMAN, RD, CDE, CD is a Registered Dietician with The Health Center in Plainfield, VT where she has spent the past 30 years providing clinical dietary care to patients throughout the region, which includes her work as a certified diabetes educator. She is an active member of Green Mountain Nutrition Associates and is currently working with the Vermont Department of Aging and Disabilities, Mayo Healthcare and BCBS of Vermont. She was recognized as Outstanding Dietician of the Year by the Vermont Academy of Nutrition and Dietetics in 1998. She received her Bachelor of Science in Foods and Nutrition from Framingham State College in Massachusetts.

TERRY YONKER, RN, MS, FNP-BC is Telemedicine Clinical Care Coordinator at Finger Lakes Community Health, a migrant and community health center composed of eight federally-qualified health centers in rural New York that uses teledentistry, telemental health, telepsychiatry, telepeds neurology, teleHIV care, teleNutrition, and diabetic retinopathy screening to improve access and outcomes for patients. Previously, she was clinical lead in the telemedicine program at Rochester (NY) General’s rural health system.

COREY ZEIGLER is Director for the North-Country Health Information Partnership for the Fort Drum Regional Health Planning Organization in New York State, which has implemented electronic health records in 95% of primary care offices in the region. The community has the highest concentration of Patient-Centered Medical Homes in the United States. He previously served as the Chief Information Officer for Canton-Potsdam Hospital in Potsdam, NY, and as manager of a Battle Simulation Center on Fort Drum.
