Health Systems and Manufacturing Production Systems Exam Date: Monday, September 21, 2015 (4 hours from 1:00 p.m. - 5:00 p.m.)

Human Factors and Ergonomics Area Exams – Exam written questions will be e-mailed to registered students by 4:00 p.m. on Friday, November 6, 2015 and must be returned by 4:00 p.m. on November 13, 2015. Oral examination dates and times to be determined by ISyE faculty advisors.

Registration deadline is 4:00 p.m., Friday, September 4, 2015.

If you wish to take any of the qualifying exams listed below you need to take the following steps: 1. Print this announcement. 2. Indicate below which exam you plan to take and sign the form. 3. Obtain the signature of the faculty member who has agreed to be your PhD advisor. 4. Return the form to the ISyE Graduate Student Services office (Rm. 3186 ME) no later

than 4:00 p.m., Friday, September 4, 2015.
