I. Catalog Description

Course Prerequisite: HCA 480

Additional students Requisites: Students must have an e-mail address and internet access to Beach Board. Students must complete a minimum of 120 hours of structured work experience in an instructor approved health care organization, under the direct supervision of a preceptor-employee. Career development planning, job search and networking will be covered.Letter grade only (A-F).

Placement Site: This course anticipates you are continuing at the same internship site placement you began with in the HCA 480 course. Please contact the instructor by e-mail above, well before the first day of class if you will be changing your internship site.

MPR #1 Sunday Sep 30 MPR #2 Sunday Oct 28. If you complete 120

hours by this date, you can request I combine MPR #2 into a blended grade for #2 and #3, but the MPR should contain at least double the meaningful discussion as would a single MPR.

MPR #3 Sunday, Dec 2. However, if 120 hours are not accumulated by class #7, an incomplete grade can be requested. Discuss this by class #6 with the instructor. If an incomplete is supported by the instructor, delay the MPR #3 and Preceptor’s final evaluation until you complete 120 hours and then turn in those two final assignments, (MPR #3 and the Preceptor’s Final Evaluation)

A = 142 - 158 points = Exceptionally OUTSTANDING work, includes a great deal of depth of reflection on the topics under discussion, use of research citations and or other significant contributions, is far beyond basic requirements, far exceeds standards on rubrics posted in the BB course content section.

B = 126.5 – 141.5 points = SUPERIOR work, far exceeds standards listed below, includes a great deal of reflection on assigned topics, includes several meaningful contributions beyond the basic instructions, far exceeds minimum meaningful word count

C = 110.5 - 126 points = GOOD work, covers all the items in the assignment, polished, easily understood, contains no syntax, spelling, punctuation or grammatical errors. Follows all the instructions, meets the minimum suggested word count. Viewed as getting the work done.

D = 95 - 110 points = Below ACCEPTABLE work, fails to cover all assigned topics, contains grammatical, syntax, spelling, punctuation errors, all instructions are not followed, does not meet suggested minimum meaningful word count, work does not meet stated requirements.

F= <95 points = Unaccepted work submitted, and/or assignments not submitted, or submitted past the not later than due date without instructor being notified at least a day in advance of the due date, without an acceptable excuse as defined by CSULB policy.

There will be no curving or rounding up of final grades.

VI. Assignment Detail Description & Instructions

All assignment templates and forms are available to be downloaded from the course BeachBoard Content section. Please submit all assignments into the designated Dropbox correctly labelled mailbox folder on BeachBoard on time. The Dropbox folder will close at 11:59 PM on the day the assignment is due. Late assignments will not receive grading point credit.

1. Student Contact Information SheetPlease provide your most current contact information, due not later than the first day of the class in the instructor’s drop box (DB). Type and fill out the form completely

2. Preceptor Contact Information and Preceptor-Student Learning Agreement form

Fill out the student section of the form fully. Target the type projects you want to obtain and polish, expertise and skills you want to develop. The more effort you put into

a concise and thoughtful summary, the better the experience you will have at the site. Then make an appointment with your preceptor, bring two copies of your form to begin the discussion of what the preceptor will assign you to perform. Once the preceptor makes inputs and you both sign, send a copy to the instructor’s drop box. Note, you will not be able to start accumulating internship hours until this form is received by your instructor. Also, the instructor will check on the CSULB Affiliation Agreement status. IF a current agreement does not exist, a renewal form will be sent to you. Students CANNOT begin internships unless an approved affiliation agreement is in place.

3. Instructor’s Dear Preceptor Letter. Find this letter with the forms at the bottom of the content section. Give a copy to your preceptor. It lists expectations during the internship of the student, preceptor, and instructor.

4. Cover Letter, ResumeAs will be briefed in class, students must attend a resume review workshop before Sep 25, provided by the Career Development Center or visit the Writer’s Resource Lab. A validation certificate should be obtained and submitted to the drop box labelled “Final CL and Resume”. The validation certificate is located with course forms in the content section. Students should use comments from instructor’s lecture in class #1 and advice from the review workshop to polish their resumes and cover letters. Students will prepare updated hard copy drafts of these documents and bring them to class #3. (SMART phone or computer lap top presentations of these docs are not acceptable). An ICE of groups of 4 and the instructor will perform reviews of these docs. to use to further improve their docs. The final resume and CL submission to the DB is due NLT Nov 4.

5. Job Search PlanThe plan developed and used by the CSULB Career Development Center, will be used as a model and assignment guide to help students develop career plans. This model includes the following elements P = Personal Assessment, R = Research Careers or Graduate Schools, O = Organize findings, C = clarify career options, E = Evaluate Findings, S = Select Options and the final S = Set into Motion which will devise the students “Job Search Plan”. Each letter of the plan will have an assignment and deliverable associated with it. Instructions and forms for the plan components are listed on the "assignments" page. The Job Search Plan will be the concluding step of the “Process” Model. It will include a step-by -step description of how the student intends to search for a job placement after graduation.

6. Professional Association Meeting, or Interview 5 HC Executives on networking tips and value added certifications. Report is due in DB NLT Nov 4, and presentation in class #7.

7. Business Card.Directions for preparation of a personal professional business card to begin your networking and job search process will be covered in class and examples provided for guidance.

Rubrics, student samples with grades of A, B and D will be reviewed in class. This monthly report covers what projects you are working on, surprises encountered, reflections on value added and non-value added processes observed. Also, a skill assessment matrix will help target the skills you need for your targeted career job and what steps you are taking to obtain the expertise. In order to receive full credit for the assignment, the report must be TYPED, signed by your preceptor and submitted to the Dropbox by the deadline. Late submissions will not be accepted. Follow the guidelines posted on Beachboard, after each section please includes the word count

9. Job and Graduate School references.Review of documents the student needs to create to help the reference person write a targeted, effective reference letter.

10. E-Portfolio and Career Objective Statement Video Students will present these updated documents in class #7 using their lap top computers in an ICE group of 4 and the instructor. You will need to set appropriate privileges to allow instructor or others to view your e-Portfolio. Please test this thoroughly prior to submitting your presenting product. If the instructor cannot view areas of your e-Portfolio you will receive zero credit for that portion of the project.The following elements are required for the HCA 481 portfolio (Bold items are additions or updates from HCA 480 portfolio):

(1) Navigation Bar (not limited to but, including the following links to pages)(2) Updated Professional Objective Statement (short video) a 30 second to 3 minute

11. Student FeedbackPlease provide your feedback on satisfaction with the site, the preceptor, and topics covered in the course

12. Preceptor Evaluation & Telephone Exit InterviewExit survey with the preceptor to discuss your activities, learning and performance done via a phone call lasting approximately 8 - 10 minutes. The student should be with the preceptor during the call. A sign-up list schedule will be circulated in class. This call takes place at the end of your internship. If you have classmates at the same site and

want to do the call together, this is acceptable as long as you understand and approve your strengths and opportunity areas for improvement will be discussed by you and your preceptor in front of your classmates during the call. Reserve your day/time on the sign-up sheet. Students must originate this Telcon. If the call does not take place because of an error on your part then the call will not be re-scheduled and points will be lost for this assignment. Plan ahead and schedule an early call. Every semester I’ve seen students schedule late, obtain a Telcon appointment time 1-3 days before the deadline and their preceptor then goes on vacation or to a conference and is not available. The student receive zero points for failure to adequately preplan and not accomplish the Telcon.

13. Preceptor’s End of Course EvaluationStudent must print out this form from course content, place their name on it and give to their preceptor to complete. It is due NLT Dec 9 in the DB. I suggested you have it completed earlier in case the preceptor suddenly goes on vacation or is not available. The course ends this week.

VIII. HCA 481 Course PoliciesA. Written assignments grading- Under the assignments section listed above are the

maximum grades that can be given for each specific assignment. The grading of each assignment will be dependent on the depth of reflection and meaningful discussion provided. This will also include following instructions and directions, clarity in the presentation of ideas, grammar, spelling, syntax, reference to discussions in the classroom, outside research citations provided, and interviews conducted. A total word count (i.e. TWC 582) needs to be listed after the last sentence at the bottom of the document submitted. The student’s name and HCA 481 and course section number should be placed on the first page of each submission. Students should familiarize themselves with the grading rubrics and the sample student reports posted in content.

B. Assignments due dates – All assignments are due on or before due dates. There will be no credit (grade of zero points) given for late submissions. If you have a justifiable emergency, let the instructor know in advance of the due date of the emergency situation. This policy has been adopted by all the HCA Internship instructors.

C. Checking BB- Students are responsible for checking the course on BB several times during the week for update memos from the instructor, and checking the grade box to make sure credit is received for all course materials submitted in the drop box on time, and in the appropriately titled mail box.

D. Class attendance- Each student is required to attend each class meeting, be on time, fully participate in the entire class meeting in order to receive credit and not leave early. For those missing all or portions of class meetings, they must meet with classmates to obtain updates on material missed. Students with excused absences per university policy should contact the professor within 3 days of the absence to receive a make-up assignment. All approved make up assignments are due prior to class # 7 for credit. Students must sign the Class Sign-In Log during each class session to validate

attendance. A signature on the Sign in Log will be the basis if the student was in attendance.

E. Presence of documents and the ability of the instructor to open those documents in the drop box, appropriately labelled mailbox will be basis if material has been submitted and received for grading. All written assignments must be sent to the drop box, appropriately labeled mail box. For video documents submitted, please ensure each individual document has been given electronic permission for the instructor to open. Students should check the grade box to make sure a grade was given within a few days after submission. If there is no grade after one week, re-submit the document. Even though the student’s e-mail may show the document was submitted, sometimes the document doesn’t appear in the drop box or may have been sent to the wrong mailbox. Please ensure documents are submitted right side up for ease of review. Also, if smart phone pictures are sent, pictures must be entirely in focus and of font size 12 or greater that can be easily read.

F. Incomplete Grade, Withdrawals. - If for circumstances that are beyond the student’s control, and they cannot complete the internship during the semester for which they have registered, a grade of incomplete can be requested, if a minimum of 60 internship hours have been accumulated. This will be converted to a letter grade as soon as the 120 hours are accumulated and written paperwork is accomplished. If at least 60 hours are not completed, the student should request a withdrawal from the course before the CSULB deadline and re-enroll the following semester. This should be done at least 4 weeks before the last class meeting, otherwise the request will not be accepted by enrollment services for processing. Note, students must initiate this action to withdraw from class. Students requesting an incomplete must complete all the course assignments before the end of the last class meeting. The only exceptions are that the preceptor’s end of semester evaluation, MPR # 3, along with the preceptor’s verification signature that 120 or more has been accumulated. These should be then completed once the 120 hours are accumulated and send the instructor an e-mail with these three documents, requesting your incomplete grade be converted to a letter grade. Note that once the formal class is completed, items in the course content section cannot be accessed by the student. Therefore, the student that is planning on requesting an incomplete, should print out or retain an electronic copy (before the last class meeting) of the preceptor’s end of semester evaluation form, and MPR #3 format.

G. AccommodationStudents needing special consideration for class format and schedule due to religious observance or military obligations must provide the instructor with written notice of those needs.

Students who require additional time or other accommodation for assignments must secure verification/assistance from the CSULB Disabled Student Services (DSS) office located at 270 Brotman Hall. The Telephone number is (562) 985-5401.

Accommodation is a process in which the student, DSS, and instructor each play an important role. Students contact DSS so that their eligibility and need for accommodation can be determined. DSS identifies how much time is required for each exam. The student is responsible for discussing his/her need with the instructor and for making appropriate arrangements. Students who are eligible to receive accommodation should present an Accommodation Cover Letter and a DSS Student/Teacher Testing Agreement Form to the instructor as early in the semester as possible, but no later than a week before the first test. It takes one week to schedule taking an exam at the DSS office.) The instructor welcomes the opportunity to implement the accommodations determined by DSS. H. Cheating and Plagiarism. Please be aware of and ensure that your behavior conforms to University Policy. See http://www.csulb.edu/divisions/aa/grad_undergrad/senate/documents/policy/2008/02/Although the University catalog does not cover this aspect of plagiarism, please be aware that it is NOT acceptable to submit the same paper for two courses. If you want to write a paper on the same topic area for two different courses, you must submit two different papers. If I discover that you have submitted the same paper for another course, you will receive a failing grade for your paper in this course.

I. Withdrawal policy. Per University policy: http://www.csulb.edu/divisions/aa/grad_undergrad/senate/documents/policy/2002/02/. Withdrawal after 2nd week and before final 3 weeks “permissible for serious and compelling reasons,” instructor will evaluate student withdrawal requests on a case-by-case basis.

J. Campus Behavior “Civility Statement-Civility and mutual respect toward all members of the University community are intrinsic to the establishment of excellence in teaching and learning. The University espouses and practices zero tolerance for violence against any member of the University community. A threat of violence is an expression of intention that implies impending physical injury, abuse, or damage to an individual or his/her belongings. All allegations of such incidents will be aggressively investigated. Allegation that are sustained may result in disciplinary action up to and including dismissal from

K. Classroom Expectations—All students of the California State University system must adhere to the Student conduct code as stated in Section 41301 of the Title 5 of the California code of Regulations as well as all campus rules, regulations, codes and policies. Students as emerging professionals are expected to maintain courtesy, respect for difference, and respect for the rights of others.

Unprofessional and Disruptive Behavior-It is important to foster a climate of civility in the classroom where all are treated with dignity and respect. Therefore, students engaging in disruptive or disrespectful behavior in class will be counseled about this behavior. If the disruptive or disrespectful behavior continues, additional disciplinary actions may be taken.

Class assignments will be due at 11:59 pm on the due date stated in the syllabus unless otherwise stated by the instructor. Once the instructor has collected the assignment or the drop box has closed, all other assignments are considered late. Late work is not accepted unless there is a university accepted excuse. Students may have a valid reason to miss a class. When an excused absence (see below), directly conflicts with class meeting times, students are responsible for informing the instructor of the reason for the absence and for arranging to make up missed assignments, tests, quizzes, and class work insofar as this is possible. To receive full credit for any work that is turned in late, due to an excusable absence, it must have documentation of excusable absence such as medical excuse, funeral program, flight tickets, etc. (or copies of). Non-emergency doctor’s appointments are not excused. Assignments/papers are NOT accepted by e-mail. Assignments submitted to the incorrect drop box will receive a “0” on that assignment.

L. Excused AbsencesThe university policy defines excused absences as 1) illness or injury to the student; 2) death, injury, or serious illness of an immediate family member or the like;3) religious reasons (California Education Code section 89320); 4) jury duty or government obligation; 5) University sanctioned or approved activities (examples include: artistic performances, forensics presentations, participation in research conferences, intercollegiate athletic activities, student government, required class field trips, etc.) (CSULB Catalogue, 2010). To receive credit for an excused absence you must submit documentation to the submission folder. Emails do not count. You only need to contact the instructor about an absence, if it is on the day of an exam.

Laptop and Cell phone policy - Cell phones to be turned off and out of view during class sessions. Laptops will be used on occasion for in class exercises, otherwise turned off and out of the way. Violation of this policy will result in 1. First time per semester "verbal warning”; 2nd Time excused for class session that day and written warning; and

3rd time per semester 5 points deducted from course score and each violation thereafter.

E-Cigarettes: Are not allowed in the classrooms.

Leaving class early will result in an absence for the day unless the student has made arrangements with the instructor in advance.
