Each UnitedHealthcare Children’s Foundation (UHCCF) medical grant provides so much more than financial assistance: it gives hope for a bright future. Our UHCCF grant kids and families are why we work hard each and every day. That's why we continue to add more creative ways to fundraise for our mission of improving children’s lives and helping families gain access to the care they need through medical grants.
IN 2017, WE SAW FANTASTIC GROWTH THANKS TO OUR WONDERFUL SUPPORTERS:
• 2,606 medical grants were awarded — worth $4.7 million • We hosted 17 regional events, including three brand new events
- The Tour de Triangle, presented by SAS, in Cary, North Carolina, raised $62,500
- The Steppin' Up for Kids virtual walk raised more than $72,000 nationwide
- The Minneapolis, Minnesota Holiday Boutique pop-up shop on the U.S. Bank Stadium field saw 26,000 visitors and raised over $22,000
• The Hazeltine Golf Classic in Chaska, Minnesota, raised an amazing $1.7 million in donations
• Many new UHCCF products were created and are sold on Amazon, including our fourth Oliver & Hope® storybook, three joke books written by kids for kids, a unique UHCCF PopSocket, and an Oliver & Hope coloring and activity book
• The UnitedHealth Group Employee Giving campaign raised more than $3.2 million, enough to award 1,800 medical grants for kids.

Granting Hope. CHANGING LIVES.
With her bright smile and personality, you’d never know Fiona was born with megalencephaly-polymicrogyria-polydactyly-hydrocephalus syndrome, a rare genetic disorder caused by a malformation in her brain. Her disorder led to epilepsy, speech delays, and severe mobility impairment. With a UHCCF medical grant, Fiona’s family was able to purchase a medical stroller and wheelchair. We’re happy to say she’s now taking her first steps with a gait trainer and is focused on her next big goal of having fun at nearby parks and beaches all summer.

AnuheaFor a sweet girl like Anuhea, her speech impediment wasn’t enough to take away her smile. However, her need for speech therapy outside of school came at a great cost to her family. Fortunately, with a UHCCF medical grant, Anuhea was able to receive therapy that helped her communication and comprehension skills. Most importantly, it helped her make new friends.
RENTON, WASHINGTON | AGE 8
PrestonPreston loves sports, but when his Type 1 diabetes tried to slow him down, UHCCF helped ensure he stayed in the game.
Before receiving much-needed assistance,Preston would skip meals and tell his mom he wasn’t hungry to avoid being poked 10+ times a day to check his blood sugar levels. The UHCCF medical grant enabled Preston to get a sophisticated insulin pump and blood glucose monitoring system — allowing him to focus on being a kid again.

CarterWhile Carter loves being in the water, his first months were anything but smooth sailing. Within his first two weeks of life, Carter was diagnosed with Cystic Fibrosis, gastroesophageal reflux disease, and a milk allergy. A UHCCF medical grant gave Carter access to specialists and special formulas — allowing him to get better and do what he loves: splashing around and working on his impressive sandcastle-building skills.
MIDLOTHIAN, VIRGINIA | AGE 1
JewelAs an accomplished violinist and lacrosse player, Jewel is a thriving teenager. As a newborn, however, she experienced the challenges that come with a unilateral cleft lip and palate. Thanks to a UHCCF medical grant, Jewel recently received the necessary corrective surgery — fixing her condition and boosting her self-confidence.

Over the past 10 years, UHCCF has awarded more than
15,000 medical grants — valued at more than $40 million
— to children and families nationwide. These much-
needed grants have helped cover treatments associated
with cancer, diabetes, autism, Down syndrome, cerebral
palsy, and other health conditions.
These children are in a better place today because of
the support and generosity of UnitedHealth Group
employees, donors, individuals, and corporations.
We send a special thank you to all the volunteers
who made our 2017 fundraising events a success.
We couldn't have reached this important
milestone without you.
Together we helped 2,606 children andfamilies through the medical grants program.

UNITEDHEALTH GROUP HEROES
Leave it to over 100 volunteers and thousands of participants at UnitedHealthGroup, Optum and UnitedHealthcare to rise to the occasion. With their help, UHCCF was able to launch our employee Hero Days event at over 100 sites across the nation, many included grant recipients as special guests, like Athulya, age 9, from Prior Lake, Minnesota. More than 7,000 employees donated financially to our grant program.
HERODays 2017
Last spring, people all across the nation took off on solo ventures and team treks to walk and raise money for UHCCF’s first-ever Steppin' Up for Kids virtual walk. The end result? Over 1,300 people walked and raised over $72,000! A huge thank you to all our fundraisers. It was fun to receive so many photos from your participation, too. We hope to see you out there May 1st through June 15th, 2018.

Tour de TriangleOn March 25th, cyclists in Cary, North Carolina, rode and raised over $62,500 at our inaugural Tour de Triangle Century Ride fundraiser. A special thanks to all our riders, donors, and great sponsors like SAS and Jarrett Jedlicka for planning this great event. UHCCF grant recipient, Max and his family were on hand to cheer on all the riders.
Frosty FatHeld on January 21st, the 1st Annual UHCCF Frosty Fat 40K Fat Bike Ride sent riders over the snowy, groomed trails of Hopkins, Minnesota — raising over $20,000 for UHCCF medical grants. Special thanks to UHCCF supporters Tony Wirz and Jeff Meyerhofer, the riders, sponsors, and other supporters for making this event a big, fat success.

Ride Into SpringUHCCF's Ride Into Spring online auction was a big success. Thank you to Rally Cycling for donating eight Diamondback bikes custom-inspired by UHCCF grant recipients that were auctioned off. Each was completely unique and shared the children's favorite themes. The auction raised more than $79,000 for UHCCF medical grants!

Pro Cyclist for the DayEach year, the UnitedHealthcare Pro Cycling Team raises funds for our Foundation and honors a UHCCF grant recipient as the “Pro Cyclist for the Day” at one of their races. This year, 8-year-old Adelynn from Corona, California, was honored and presented with a team jersey, helmet and a bike during Stage 7 of the Amgen Tour of California race in Pasadena.
Velo & VinesOver 60 cyclists and hundreds of participants came together for the 3rd Annual Velo & Vines Century Ride in Healdsburg, California. The ride was led off by 14-year-old grant recipient Kaelyn, from Rexburg, Idaho, brought riders through beautiful wine country as they rode their way to raising over $165,000 for UHCCF medical grants. Special thanks to presenting sponsor SalesForce, sponsors Arnot-Roberts and TRPN, and to Jennifer and Nathan Roberts for making this event so special. Join us for the next Velo & Vines event on Saturday, June 9th, 2018.
MinnesotaCentury RideOn July 30th, the 6th Annual UHCCF Century Ride, presented by CA Technologies, proved to be a huge success – raising over $625,000 thanks to our 140+ riders, hundreds of donors, and fantastic sponsors. This event's fundraising efforts enabled us to fund nearly 300 UHCCF medical grants.
After the ride, we hosted our first-ever Optum Circle After Party at the Optum headquarters in Eden Prairie, Minnesota, sponsored by Availity. Riders and supporters celebrated a great ride with food trucks, live music from Bluewater Kings, kids’ activities sponsored by the YMCA, and even a silent auction. Thank you to Jeff Meyerhofer, Tony Wirz, and the UnitedHealthcare Training & Development team for making it all happen flawlessly. Join us for the next Minnesota Century Ride on Sunday, July 29th, 2018.

UHCCF Golf Classicat Hazeltine The 8th Annual UHCCF Golf Classic at Hazeltine in Chaska, Minnesota, home of the 2016 Ryder Cup, was a huge success thanks to 240 golfers, 60 sponsors, and our incredible donors. In total, we raised an astonishing $1.7 million for medical grants for children.
Beyond golf, participants enjoyed live music by Pelican212, exotic cars provided by Morrie’s Luxury Auto for the hole-in-one contest, wine tastings by Arnot-Roberts, a live and silent auction, and the Winner’s Choice raffle. Thanks to our emcee, Alicia Lewis, KARE11 news anchor, our sponsors and incredible grant families who joined us for this special day. Join us for the next UHCCF Golf Classic on Monday, July 31st, 2018.
We are proud to have our four grant recipient heroes from the 2017 Hazeltine event as our 2017/2018 ambassadors! Ten-year-olds Tatiana (Las Vegas, Nevada), Juliet (Seekonk, Massachusetts), Noah (Clermont, Florida), and Preston (Cypress, Texas) will participate in numerous 2018 UHCCF events!

UHCCF LabCorp Golf Classic In its 8th year, the UHCCF LabCorp Golf Classic, presented by TeleTech in Rockville, Maryland on Monday, October 16th, 2017, was in peak form. With over 120 golfers teeing off at Manor Country Club, and bidding on great silent auction items, the event raised over $175,000. Grant recipients Larissa, age 7 and Kyle, age 8 both from Randallstown, Maryland and their families were on hand to cheer on the golfers as they played hard to help improve the lives of many more families.

Northeast Golf ClassicOn September 26th, UnitedHealth Group leaders, sponsors, volunteers, grant recipient Juliet age 10, from local Seekonk, Massachussetts, and supporters headed to Lake of Isles Golf Club and Resort in North Stonington, Connecticut, for the 6th Annual UHCCF Northeast Golf Classic. By the end of their rounds, they managed to raise over $95,000 for medical grants for the region.
Special thanks to Miss Connecticut 2017 Eliza Kanner, actor Paul D’Amato, Commander Richard L. Iannucci, emcee and UHCCF northeast regional board member, Bill Golden, Ed Skopas, Stephen Farrell, Jay Putterman, and Pete Bompane.
Mississippi Golf ClassicGolfers from across Mississippi gathered on October 16th, at the Dancing Rabbit Golf Club in Choctaw, Mississippi, for a day of golfing and fundraising. Participants were rewarded for their hard work on the links with a post-tournament awards luncheon and live auction. The annual event raised more than $35,000 for medical grants.
A big thanks to Joe Ochipinti, Gulf Coast Plan CEO, for his leadership and efforts. Also, Stan Neese and Ayn Cook contributed greatly to make it a very successful event.

Holiday Boutique Our UHCCF Holiday Marketplace at the Minneapolis, Minnesota, Holiday Boutique held at U.S. Bank Stadium was visited by more than 26,000 people — raising over $22,000 for the Foundation.

Florida Fishing EventLast May, Patrick Stamm, Don Reilly, and the Florida planning team reeled in $126,000 for UHCCF medical grants. Our annual Florida Fishing Event featured 13 boats, 14 vendors, and 56 attendees to the benefit of many families.

Teddy Bear Run Summerlin, Nevada, really showed its soft heart during our annual Teddy Bear Run on October 21st. This year, our fun 5K and 1-mile walk drew over 450 participants and raised over $19,000 for medical grants. Having grant recipients Karlee, 13 and Maximus, 12 from nearby Henderson, Nevada, there made the day that much better.
Brewsiana Bash Raising a glass to raise over $25,000 — we'll drink to that any day! At our first-ever Brewsiana Bash at Abita Brewery in Covington, Louisiana, over 50 people showed up to enjoy great Abita beers, Sweet Daddy’s BBQ, live music, and a silent auction.
Parents of three-year-old grant recipient, Mia, were there to share her story. Special thanks to Gulf Coast Plan CEO Joe Ochipinti and his wife Deirdre for hosting the event.
Night Outwith Our Heroes Rock & Reilly’s Rooftop in NYC was rocking during our Night Out with Our Heroes event in May — raising nearly $150,000! Over 300 supporters and special guest Boomer Esiason had fun raising money and celebrating our grant families. Special thanks to all our sponsors, UnitedHealthcare employees, Scott Lydon, Kimberly Santillo, Bill Golden, and Michael McGuire for organizing this event.

Our Grins2 program is twice as fun for kids. Because when donors purchase a Grins2 gift bag, it actually impacts two children. One child receives the gift while the other has his or her life changed by a UHCCF medical grant. One simple gift. Two very big smiles.
Each Grins to Go gift bag is filled with an assortment of popular toys and activities designed for boys and girls between the ages of 5 and 12.
In April, our Grins2 campaign raised nearly $28,000 for UHCCF medical grants — impacting nearly 1,000 children at 235 hospitals and Ronald McDonald Houses.
Our summer Grins2 campaign brought many needed smiles and laughter to more than 350 children at 128 hospitals across the nation.
In December, 173 Grins to Go activity bags and 201 Oliver plush bears shipped out just in time for Christmas. Because of your generosity, a total of 374 children received a gift during the 2017 holiday season.

the many generous people who donated these items. Proceeds from these sales help fund UHCCF

317 children's hospitals and Ronald McDonald Houses across the nation.

GermanyAt UHCCF, we’re extremely proud to see how far Germany has come. When we first met her in 2014, she had recently been diagnosed with chronic fatigue, fibromyalgia, a leg tumor, and a low-grade tumor in her cerebellum, which made it difficult for her to sleep. Today, she is all smiles and living life to the fullest.
As one of our 2017 grant recipients, we’re happy to report that Germany is currently pursuing her dream of becoming a nurse. After spending so much time in hospitals, she was inspired by the great nurses who took care of her. Now, she wants to be there for other children in need so she can make them feel better too. Beyond nursing, Germany loves to draw and sing — talents we’re sure her future patients will love.

Twins Anna and Kate share everything from looks toeven the same laughter. Unfortunately, they also share Apraxia of Speech — the disconnect from the brain to mouth that makes speech difficult. After receiving UHCCF medical grants, Anna and Kate were able to get the frequent, intense speech therapy they needed to focus on the fun things in life.
For Anna, that’s dancing. You can find her twirling, leaping, and expressing herself all day, any day. Her natural ability as a dancer is truly a gift for her family to watch. Kate is equally active, though she prefers the tumbling mat. Through flips, tricks, handsprings, and back tucks, she’s gained the confidence she needs to master anything she wants. Both are inspirations to us all.

"When my son, Bentley, was two years old, I noticed a severe speech delay. I brought him to Children’s Hospital of Atlanta where they recommended he start speech therapy. Unfortunately, with our insurance not covering speech therapy and Bentley already in feeding therapy for his refusal to eat, I literally didn’t know how I could afford it.
Thankfully, UHCCF awarded us a $1,000 medical grant for his speech therapy. I’m proud to say that Bentley is now six and his speech is doing well. I can now understand him and his teachers and doctors have noticed a big improvement. Thank you for everything. We will be forever grateful!"

"My daughter, Julia, has achondroplasia — the most common form of dwarfism. Since her birth, we have reached our $4,000 deductible every year due to MRIs, multiple surgeries, and associated tests.
We just want to say how incredibly thankful we are to have received multiple UHCCF medical grants throughout the years. Not only has each grant provided the financial support our family needed, it has helped us worry less."

"Our 5-year-old daughter, Winnie, has had Type 1 diabetes since 16 months old. As parents of a young child with Type 1 diabetes, we must constantly be monitoring our child for symptoms of hypoglycemia or hyperglycemia. Being so young, she did not know how to tell when her blood sugar was high or low.
With the UHCCF grant money, we purchased a Dexcom continuous glucose monitor — a small device that inserts a thin wire under Winnie’s skin to measure her blood glucose 24/7. This tool has been invaluable for us and her teachers. But, for Winnie, it gives her the freedom to be a kid. We cannot thank you enough for that."

How to Make a Contribution
The UnitedHealthcare Children’s Foundation offers medical grants that provide financial relief for families who have children with medical needs not covered, or not fully covered, by their commercial health insurance plan. Families do not need to have UnitedHealthcare insurance to be eligible. Please help us give the gift of hopeto families in need. Visit UHCCF.org to reviewthe application criteria and how the grant process works.
How to Apply for a Grant
Your donation will help fund medical grants in your region that help children access new or continued medical treatment. Anyone interested in donating and supporting UHCCF's mission can visit UHCCF.org to make an online contribution, or you may send us a check, payable to UnitedHealthcare Children’s Foundation, MN017-W400, P.O. BOX 41, Minneapolis, MN 55440-0041.
If you are a UnitedHealth Group employee, you can give any amount on a one-time or recurring basis through payroll deduction, credit card, or PayPal at giving.uhg.com. JerryAt age 3, Jerry was diagnosed with an oral sensory disorder that required occupational therapy. During his therapy, doctors discovered numerous underlying motor skill concerns, and recommended physical therapy as well. A UHCCF medical grant helped Jerry’s family ease the financial concern of his ongoing therapies and orthotic shoe inserts. Today, Jerry is your typical 6-year-old, bike-riding, jump-roping, fun-loving boy — just as he should be.

Since day one, Dominic has been a fighter. Born at just 29 weeks and weighing only 2-lbs., 7-oz., Dominic has already seen his fair share of battles. Diagnosed with cerebral palsy, cortical visual impairment, epilepsy, and periventricular leukomalacia, he requires a wheelchair and other specialized mobility equipment to get around. With a UHCCF medical grant, his parents were able to afford his first wheelchair, which gave him the freedom to be a kid and enabled them to take him places more easily.
Today, Dominic is a happy, fun-loving little boy who loves music, super heroes, reading, being with friends at school, and hippotherapy — where he gets to ride a horse! He even participated in the Hamilton County Special Olympics track and field spring meet, where he competed in the 10-meter assisted walk. Way to go, Dominic!
