Important message for the Parents of 5th Grade Students: This is a reminder that Arizona law requires that all incoming 6th grade students have proof of the following vaccinations by the first day of school for the 2014-2015 School Year: 1. Tdap – 1 dose is required when 5 years has passed since the last Tetanus shot and the student is at least 11 years old. 2. Meningococcal – 1 dose is required for all incoming 6th graders, 11 years and older. A reminder of immunization needs for fifth graders will be sent out with your child’s final report card in late May. If you have any questions about the vaccination requirements please contact the CV Health Office @ 209-7704. If your child receives these immunizations before the end of the current school year, please bring in the documentation to the health office at Canyon View so that we may update our records. If your child receives these immunizations over the summer, they may be turned in to Esperero Canyon Middle School front office, faxed to 209-8170 or emailed to [email protected]. minimum bid increments which are listed on the bid sheets. Please write your last name and first initial or a phone number. Read item descriptions carefully, including expiration dates. 5. BUY IT NOW (BIN) items can be bought during auction hours by taking the item’s bid sheet to Check-Out, paying the BIN price listed in red on the bid sheet, and presenting the sheet marked “paid” to Pick-Up to receive the item.

6. There is no limit on the number of items one person may bid on or buy. 7. Check-out will begin approximately 15 minutes after silent auction closing
