A HRSA EXAMPLE (Was due Nov. 14): • Rural Health Care Services Outreach Grant Program (Outreach Program).    ORHP’s Outreach

Program is a community-based program targeted to improve outreach and service delivery in local and regional rural communities.  

• The goals for the Outreach Program are the following:

1.      Expand the delivery of health care services to include new and enhanced services exclusively in rural communities

2.      Deliver health care services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services

3.      Utilize and/or adapt an evidence-based or promising practice model(s) in the delivery of health care services

4.      Improve population health, demonstrate health outcomes and sustainability

Requires a Consortia Model of 3 or more entities

Rural Appalachia will have an advantage

HRSA – Maternal Child Health (due Dec. 5th)• The R40 MCH Research Program supports applied research relating to

maternal and child health services, including services for children with special health care needs. 

• Research should demonstrate a substantial contribution to advancement of the current knowledge pool, and when used in States and communities should result in health and health services improvements.  

• Outcomes expected to strengthen and expand Affordable Care Act (ACA) implementation. 

• This research is meant to drive policy and service delivery of preventive and early intervention services for application in health care delivery or develop new knowledge on the impact of insurance coverage on health promotion.

BJS Chritable Foundation - $2500 grants: TYPES OF SUPPORT • Self-Sufficiency

Over the last decade, the country has seen a rise in the number of reported homeless families. And studies have shown that these families are more likely to become self-sufficient by first having a safe place to sleep and then access to other support services. Support will be given to nonprofits that work toward the self-sufficiency of their clients. Grants will fund enhancement of shelter programs, purchase of supplies and clothing for residents, as well as programming for families safety.

• HealthcareSupport will be given to nonprofits that work on providing healthcare to those who would not normally have access. Funding will be disbursed to agencies providing supplies to free medical clinics, health prevention services, as well as supplies for vision, dental and hearing screenings.
