TEAMS: 6 people per team, high school students only, $20 per person. Each player must turn in a completed release form (on back). All the money goes towards Haiti Relief Funds. See website for all the details.
FORMAT: There will be a series of round-robin games for all teams to play. Once the round-robin games are complete a tournament bracket will be put together based off of the round-robin results. So bring your A game!
POST-GAME: After the tournament is over a free concert will be held to celebrate the event.
REGISTER: Email [email protected] with the following information: team name, full name of all 6 people on your team, identify the team captain & give captains contact info (email & contact number).

Please bring this completed form with you the day of the event. At registration (on May 1) your team captain will collect these forms and
turn them in with payment for you and your team.
There is a tremendous need in this small village. Prior to the earthquake, there were more than 400 orphans in this small village. Now, there are more than 2,000 orphans. Children with no families, no hope, no food, no shelter. The money raised support a missionary (Kathleen Willet) living in the community to oversee the building of shelters, food programs, education, and medical assistance. In addition, she will be working with local pastors to share Jesus with the children and community! 20% will go to Food for the Hungry (www.fh.org). Food for the Hungry is a non-governmental organization dedicated to helping people around the world. Through a grant with the U.S. Agency for International Development, for every $1 raised, the U.S. Government will give $20 to Food for the Hungry for Haiti relief efforts.

School from 2-8 p.m.. I understand that my child will be playing competitive dodgeball and could be
injured at this event. In the event of an emergency, I give my permission for the Youth Staff to provide
any medical care they deem necessary after notifying the parent or guardian. Students will not be
permitted to leave the school campus during this event. If for some reason a student needs to leave early
then please state the time, the reason, and adult or parent that will be picking them up.

Where will the money go?100% of the funds raised through this dodgeball tournament will go to Haiti relief efforts. We will be working through two organizations to use the funds for Haiti relief. 80% will go to WILLET Missions (www.willetmissions.org). Through WILLET Missions, funds will go to small town in Haiti called Belladere. Belladare is about located about 4 hours east of Port-Au-Prince. This small village has become a refuge for Haitians fleeing the destruction in capital city.
