Introduction

This is the report of a re-accreditation audit from 03 March 2015 to 04 March 2015 submitted to the Quality Agency.

Accredited residential aged care homes receive Australian Government subsidies to provide quality care and services to care recipients in accordance with the Accreditation Standards.

To remain accredited and continue to receive the subsidy, each home must demonstrate that it meets the Standards.

There are four Standards covering management systems, health and personal care, care recipient lifestyle, and the physical environment and there are 44 expected outcomes such as human resource management, clinical care, medication management, privacy and dignity, leisure interests, cultural and spiritual life, choice and decision-making and the living environment.

Each home applies for re-accreditation before its accreditation period expires and an assessment team visits the home to conduct an audit. The team assesses the quality of care and services at the home and reports its findings about whether the home meets or does not meet the Standards. The Quality Agency then decides whether the home has met the Standards and whether to re-accredit or not to re-accredit the home.

The organisation’s quality systems effectively identify, action and evaluate continuous improvements across the Accreditation Standards. Management sources information through stakeholder feedback, analysis of audits and monthly clinical data collection, resident needs, legislative changes and strategic planning. Management logs, monitors and evaluates actions with input from relevant departments. Continuous improvement is an agenda item at all relevant meetings. Identified improvement opportunities may result in reviews of policies and procedures, equipment purchases, additional staff training and updates to the audit schedule. Residents, representatives and staff are aware of the various avenues to make comments, complaints and suggestions and are encouraged to be part of continuous improvement at the home.

Improvements over the last 12 months relating to Standard 1- Management systems, staffing and organisational development include:

Management identified the need for a comprehensive information booklet for new residents. They sought guidance from specialist consultants and researched handbooks available at other homes. The newly developed handbook provides clear, practical information on care and services at the home along with their mission statement and relevant policies relating to safety and residents’ rights.

The home’s electronic care program became outdated and unsupported by the service developer. Management sourced other available programs and installed a new system in July 2014. The program includes care management, staff messaging and monitoring reports for analysis and trending of data collection. The new system provides ongoing service support and updating of the software program.

The home notifies residents and representatives of any relevant changes to legislation.

Recruitment and orientation processes include identification of skills and the provision of education to ensure appropriate skills and knowledge. Management develops the annual education program based on the outcomes of performance reviews, quality activities, mandatory training requirements and the monitoring of residents’ care needs. Education systems include online learning and individual education opportunities for management and staff. Staff receive email notification of their education requirements, which includes a link to the corresponding online education site. Staff and management said they have opportunities to attend education and they receive support to participate in learning and development opportunities.

Information on the complaints process is in the information handbook given to residents and representatives prior to entry and is documented in their agreements. The home displays external complaints and advocacy brochures and provides access to internal comments and complaints forms and secure suggestion boxes. The home has an open door policy and regularly consults with staff, residents and representatives providing group and individual forums to raise issues or concerns. Analysis of complaint data occurs and feeds into the continuous improvement system. Residents, representatives and staff are aware of the process and documentation shows matters are actioned appropriately and in a timely manner.

Management systems ensure there are appropriately skilled and qualified staff sufficient to ensure delivery of care and services according to the home’s vision and residents’ needs. Recruitment processes include identification of necessary qualifications and skills, interview guidelines and reference checks. New staff complete a structured orientation process and work buddy shifts with experienced staff before commencing regular shifts. Roster and leave vacancies are managed using regular or casual bank staff. Appraisals, competencies and management observations monitor staff skills and practices and assess the effectiveness of the staffing roster. Staff said they have sufficient time to perform their roles and are satisfied with staffing levels. Residents and representatives are satisfied with the care provided and the availability of staff to meet the needs of residents.

1.7 Inventory and equipment

This expected outcome requires that "stocks of appropriate goods and equipment for quality service delivery are available".

Management demonstrates systems to ensure appropriate goods and equipment are available for quality service delivery. Key personnel monitor stock levels and re-ordering processes are through an approved suppliers list. Adherence to maintenance and cleaning programs occurs and electrical equipment is tested and tagged for safety. Reviewing and updating of goods and equipment reflects identified special needs of the current resident population. New equipment is trialled prior to purchase and staff receive training. Equipment, supplies and chemicals are securely stored with access restricted to authorised personnel.

Residents and staff stated adequate supplies of appropriate goods and equipment are available at all times.

Policies and procedures, education, meetings, handover, communication books, daily diaries and position descriptions inform staff. Key data is routinely collected, analysed and is available for relevant staff to review. Electronic records are password protected with restricted levels of access. Resident and staff files are stored, archived and destroyed securely according to regulations. Staff, residents and representatives are satisfied with their level of access to relevant information provided at the home.

1.9 External services

Contractors undertake an orientation to the home prior to commencing. The organisation regularly reviews satisfaction with externally sourced services through feedback from management, staff, residents and representatives, audits, surveys and observations. A list of preferred service providers is available and staff have access to after hours’ emergency assistance. Staff and residents are satisfied with the quality of currently sourced external suppliers.

The home actively pursues continuous improvement in relation to residents’ health and personal care. The audit schedule includes monitoring of clinical outcomes and monthly incident/infection data is collated and analysed. Identified issues result in corrective actions through the quality system. Refer to expected outcome 1.1 Continuous improvement for more information about the home’s systems and processes.

Improvements over the last 12 months relating to Standard 2 – Health and personal care include:

Staff suggested an alternative pain patch chart to track treatments and outcomes. The new sheets contain more precise information and clear documentation of daily applications. Staff feedback is positive to the new strategy for managing residents’ pain.

The home introduced an electronic program for wound monitoring to replace the previous paper based system. The program generates daily alerts and instructions for staff and produces monthly reports for trending and analysis. The new system provides improved, consistent wound management with better outcomes for residents.

Nursing staff review care plans three monthly, consulting with the resident and/or representative. Care plans describe needs and preferences and are consistent with handover information and staff feedback. External health specialists provide reports and contribute to care plan development and review. Communication systems inform staff and other health professionals of changes in residents’ care. Staff report on clinical indicators such as skin tears, falls, behavioural and medications incidents. Management monitor clinical care through clinical data analysis, care plan reviews and stakeholder consultation. Residents and representatives are satisfied with the clinical care residents receive.

2.5 Specialised nursing care needs

Residents expressed interest in learning more about other cultures following an increase in staff from other countries and relatives sharing stories of recent overseas holidays. Lifestyle staff introduced cultural days into the activity program. The special days focus on food, music and costumes from other nations with staff or relatives sharing information on the chosen country. Residents have enjoyed the days already held and staff plan to continue with the activity.

To record residents’ involvement in the activity program, staff developed photographic folders. Each resident has their own folder with photographs to keep in their room. This stimulates conversations with visitors and informs families of the different activities offered. Residents are proud of their folders and enjoy sharing them with visitors, staff and other residents.

A recent gastroenteritis outbreak identified staff would benefit from additional training to reinforce infection management practices. Staff attended compulsory sessions run by two external specialist services. Management report staff now have a greater awareness and understanding of outbreak management practices including protective clothing and equipment storage systems and their expected actions in the event of any future outbreaks.

A recent audit identified gaps in the cleaning program including inconsistent documentation of completed tasks. Management introduced a new template for cleaning staff to sign off daily which captures all scheduled duties in residents’ rooms and common areas. Management report the new strategy has resulted in improved cleaning, infection control, communication and accountability.

A staff and resident vaccination program is in place. There are supplies of protective clothing and equipment for the disposal of sharps and infectious waste. Food safety, pest control and environmental services comply with legislation and infection control guidelines. Management monitors infections and present monthly reports at relevant meetings. Staff demonstrated understanding of infection control practices. Residents and representatives said they are satisfied with the management of any residents’ infections.

An external company provide meals to the site using cook chill technology according to a seasonal menu developed in consultation with residents and dietary services. Staff determine residents’ meal choices each day and alternative meals are readily available. Effective systems ensure catering staff have current information relating to residents’ nutrition and hydration requirements, food allergies, preferences and choices.

There is a registered food safety plan, current kitchen registration and annual food safety training for all staff. Cleaning systems and schedules guide staff to complete cleaning tasks. An external company provide laundry services for all linen and an onsite laundry provides for residents’ personal items. There are systems to minimise cross infection and ensure residents receive their correct laundry in a timely manner. Residents and representatives are satisfied with the catering, cleaning and laundry services.
