Lakelands Family Health Team Denbigh address: 31 Central St Denbigh ON K0H 1L0 Phone: 613-333-1333 Northbrook address: 12357 Hwy 41 Northbrook ON K0H 2G0 Phone: 613-336-8888 Email: [email protected] Website: www.lakelandsfht.ca Location: Same as above Hours: The hours of service for both Denbigh and Northbrook are 8:30am to 4:30pm with extended hours in Northbrook on Tuesday until 8:00pm. Service Information: The family health team provides primary health care services including chronic disease management, disease prevention and health promotion at 2 sites, Northbrook and Denbigh, to rostered patients. Vacationers are also seen at the Northbrook site. Lakelands Family Health Team’s catchment area encompasses Addington Highlands, North Frontenac as well as portions of Central Frontenac, Hastings and Renfrew counties. Both sites are fully accessible. The Team consists of Physicians, Nurse Practitioners, a Chiropodist, a Mental Health & Grief Counsellor, RN/Program Coordinator, RPNs, as well as support staff.

Services include: Physicians and Nurse Practitioners provide

individuals, families, groups and communities with health services in health promotion, disease and injury prevention, cure, rehabilitation and support

Dietitian provides counselling for diabetics Mental Health Counsellor provides therapy

and other therapeutic functions for individuals, couples, families and the community

Chiropodist assesses and treats foot conditions

Registered Nurse (RN)/Program Coordinator, in consultation with the team, develops programs (hypertension, smoking cessation, chronic pain, diabetes, health living, COPD/asthma, mental health) which are offered to rostered patients

Blood work, Spirometery (breathing tests) and EKG and services offered on site at both Denbigh and Northbrook

Minor surgical procedures such as skin biopsies, suturing and splints are completed at both locations

Aging brings increased health issues and it becomes very important that you take charge of your health. Careful monitoring by you and your health team will help you make changes needed to stay healthy. It is also important to have all the tests and screenings your health team recommends. Maintaining a close, open relationship with your health team will bring you peace of mind.

Did you know? Already one of the most common cancers in the world, the incidence of skin cancer is on the rise. It has been estimated that nearly half of those who live to age 65 will develop skin cancer at least once. Luckily, it’s one of the most curable forms of cancer – but early detection is the key. Keep an eye on your skin and talk to your doctor if you see any suspicious changes in your skin, new growths, sores that won’t heal or changes in a lump or mole.

Registered Dietitian, Sharbot Lake Family Health Team Contact: Jennifer Allan, RD 1005 Medical Centre Rd Sharbot Lake ON K0H 2P0 Phone: 613-279-3312 Service Information: A Registered Dietitian (RD) is a trained food and nutrition expert who: -Provides personalized education and support on disease management, weight management, label reading, meal planning and general nutrition. -Works with other health care professionals at the SLFHT to manage nutrition for health promotion, disease prevention and treatment of acute and chronic disease. -The services of the RD are available to all patients of Sharbot Lake Family Health Team and we continue to accept new patients. -Self -referrals are accepted.

Did you know? Dartmouth Medical School researchers discovered that test subjects who took calcium regularly and faithfully for 4 years had a 36% reduction in the development of new pre-cancerous colon polyps even five years after the study ended. We all know that milk is a primary source of calcium. Three eight ounce glasses of fat-free milk, along with an 8oz. serving of yogurt or a 2-3oz serving of low fat cheese consumed daily provides the same calcium intake as that used in the study. Check with your dietitian.

Did you know? High Fibre = Less Cancer. You should eat dark not white? White bread has a high glycemic index, which means that it quickly raises your blood sugar. The same can be said of regular pasta, potatoes, white rice and sugary treats. If these are your choice, you may run a higher risk of colorectal cancer than persons who regularly eat low-glycemic-load foods, such as whole wheat pasta, sweet potatoes, brown rice and whole grain breads. A diet high in fibre and low in sugar will be amazingly helpful in weight control also.

Did you know? A small amount of unsaturated fat every day is a healthy and natural part of a proper diet. Two to three tbsp.. of oils such as canola, olive or soy will satisfy your body’s need for oil and may even help you to develop your good cholesterol, while lowering the bad stuff. Non-hydrogenated margarine, salad dressings or mayonnaise containing these oils are also good choices. Avoid butter, lard, hard shortening and hard margarines.

Service Information: A nurse practitioner (NP) is a registered nurse with advanced university education who provides personalized, quality health care to patients. Ontario nurse practitioners provide a full range of health care services to individuals, families and communities in a variety of settings including hospitals and community based clinics in cities and smaller towns in Ontario. Nurse Practitioners work in partnership with physicians, nurses and other health care professionals to keep you, your family, and your community well. As a member of the health care team, Nurse Practitioners offer a variety of health care services including the ability to: Diagnose and treat illness and /or injuries Perform physical checkups Order and interpret diagnostic tests Write prescriptions Provide counselling and education Provide supportive care during illness Provide treatment and/or procedures Make referrals to family physicians, specialists and

other health providers

Did you know? It might be better to schedule your appointments to avoid Mondays and the days following holidays when the doctors are in greatest demand. By doing this, you may spend less time in the waiting room.

Did you know? You should tell your team of health care professionals about all the medicines you take, including prescription and over-the-counter (OTC) medicines, such as pain relievers, antacids, cold medicines, and laxatives. Don't forget to include eye drops, dietary supplements, vitamins, herbals, and topical medicines, such as creams and ointments.

They work collaboratively with other health professionals to provide care that includes health promotion and illness prevention as well as diagnosis and treatment of disease. Service is available to any patient of the Sharbot Lake Family Health Team and we continue to accept new patients.

Did you know? If you are feeling stressed, you can calm yourself with a breathing and relaxation technique. Sitting quietly in a chair, with your hands face down on your legs, begin to focus on your breathing. You can sit still and focus or rock slowly back and forth, breathing in time with the rocking motion (about ten times). Then, for five breaths think about your feet and feel them relax. Then, for five breaths think about your legs and feel them relax. Then, for five breaths think about your torso and relax. Then, do the same for arms, shoulders, neck and head and feel them relax. Sit still for a few more minutes when you're done breathing and relaxing. With practice, the body soon becomes better able to relax. Joining a Tai Chi class can help make routine a habit.

Did you know? If you aren’t sure about your doctors’ information or instructions, you should ask questions. Doctors may use technical language that is not familiar to you. Ask them kindly to explain what they mean and to give you examples. Maybe they could write it down or give you additional literature, if necessary. They usually have many brochures in their office that will help explain more about the condition discussed. If you often have trouble understanding or remembering, take a family member or friend along to your appointments.

Registered Nurse, Sharbot Lake Family Health Team Contact: Cathy Fox RN 1005 Medical Centre Rd Sharbot Lake ON K0H 2P0 Phone: 613-279-2100 Service Information: Service is available to all patients of Sharbot Lake Family Health Team. .Registered Nurses (RNs) provide treatment, education, advice and emotional support to patients and their family members. RNs record medical histories and symptoms, perform diagnostic tests and assist with patient follow-up and rehabilitation. RNs teach patients and their families how to manage their illness or injury. They explain post-treatment home care needs, diet, nutrition, and exercise programs, and self-administration of medication and physical therapy. -RNs also provide general health screening, immunization clinics and flu clinics as well as well baby checkups, INRs.

-Services RNs can provide include: vital signs and blood pressure monitoring glucometer readings ear syringing ECGs, hearing and vision testing testing for pregnancy, rapid strep and urine staple/suture removal, wound care, dressing

changes stool cards for occult blood testing, drawing

blood for testing test results entry and update to patient records with

medications and allergies education to help you manage diabetes,

hypertension and other on-going medical conditions

help you with your INRs if you are taking medication to prevent blood clots

assess your 0-18 month child, support you and your newborn with breastfeeding

telephone support or in-person triage, follow-up as needed

immunizations (flu, allergy and other prescribed shots and vaccines)

A Registered Nurse Diabetic Educator (RND): Assesses, monitors and evaluates individual patient

needs and develops diabetes management plans to ensure successful outcomes

Provides individualized education and care to patients and their families about how to effectively manage diabetes

Teaches patients the skills necessary in self-care management of diabetes

Focuses on diabetes prevention, early detection and screening

Works collaboratively with physicians, registered dietitian and other team members to provide complete care for patients

Did you know? It is a myth that diabetics must avoid sugar at all costs, but what they must do for their occasional treat is to plan properly and make sure that the treat or dessert is part of a healthy plan or combined with exercise.

Did you know? You should get a good night’s sleep. Sleep plays an important role in regulating hormone, blood glucose, and cardiovascular balance and function. It decreases heart rate and blood pressure. Over the last 50 years we have tended towards shorter sleep duration, probably due to work and social pressures. This trend coincides with an increase in cases of Type 2 diabetes and obesity.

Did you know? Both diabetes and alzheimer’s disease share many risk factors, including age, gender, obesity, high blood pressure and high cholesterol levels. Diets high in fat, especially trans and saturated fats can negatively affect your ‘brain power’. Diets high in vegetables, fruits, grains and fish are connected to better reasoning and memory. People with diabetes are at risk of memory loss after eating unhealthy meals.

Respiratory Therapist Sharbot Lake Family Health Team Contact: Sue Deveau, Registered Respiratory Therapist (RRT)/Certified Respiratory Educator (CRE) Chronic Obstructive Pulmonary Disease 1005 Medical Centre Rd Box 70 Sharbot Lake ON K0H 2P0 Phone: 613-279-2100 Service information: Respiratory therapists (RRTs) evaluate, treat, and care for patients with breathing or other cardiopulmonary disorders. Practicing under the direction of a physician or nurse practitioner, respiratory therapists assume primary responsibility for recommending respiratory care, therapeutic treatments and diagnostic procedures. -RRTs, acting as Certified Respiratory Educators and specializing in the treatment of Chronic Obstructive Pulmonary Disease (COPD), perform a patient respiratory assessment, review their medical history and help determine what combination of medication, exercise, and therapy as well as education and COPD Action Plan that can improve a patient’s condition. -RRTs use specialized tools in order to measure lung capacity and oxygen saturation. They analyze blood gases in order to help determine the extent of a patient’s breathing capacity.

Did you know? For those with COPD, exercise is an important part of life. Including simple stretching and breathing exercises or a walk into your daily life will help you maintain your physical and emotional well-being. There are many benefits, because exercise: Helps you use oxygen more efficiently by strengthening the muscles you use for breathing as

well as your other muscles. Helps you maintain or lose weight. Exercise helps you control your weight, making it easier to

breathe. Improves your mood: Exercise is associated with relieving depression and helps you relax. Improves your sleep: If you sleep better at night, you will have more energy during the day.

Exercise helps all of us sleep better.

Did you know? As you age, your fillings and crowns also age, and your body produces less saliva. Saliva helps prevent decay by neutralizing acids and washing away leftover food in your mouth.

Did you know? There is mounting scientific evidence that suggests a link between poor oral health and serious medical conditions. Oral infections can cause blood sugar to rise and make diabetes harder to control. Studies point to a relationship between poor oral health and heart and blood vessel disease. Bacteria in infected gums can enter the blood stream, attach to blood vessels and cause clots, which aggravate high blood pressure and increase the chances of heart attack and stroke.

Did you know? Dry mouth is caused when the salivary glands don’t produce enough saliva. It can be very uncomfortable and increases the risk of developing cavities and gum disease. Dry mouth is a common side effect of many medications and can be a symptom of some conditions and diseases. For those suffering from dry mouth, sipping water frequently, chewing gum or sucking on ice chips and sugar free candy may help. There are also over the counter saliva substitutes available at drug stores, which may help. Using a fluoride rinse can help reduce cavities for those suffering with dry mouth, as well as others.

Did you know? You should always remember to tell the doctor or pharmacist the names of prescribed drugs, but it is also important to mention non-prescription over the counter drugs and nutrition supplements. They too may affect symptoms or cause reactions.

Did you know? If you have trouble swallowing tablets, ask your doctor or pharmacist for ideas. There may be a liquid medicine you could use instead. You may be able to crush your tablets, but don’t break, crush, or chew tablets without first asking your health care professional.

Did you know? If you are taking aspirin (plain, baby or enteric coated) for heart problems, you SHOULD NOT take ibuprofen or Advil as a pain medication. Ibuprofen counteracts the effects of aspirin for blood-thinning therapy for heart patients.

Did you know? Be sure to talk with your pharmacist when you pick up any prescription. This allows an additional opportunity to uncover a mistake that may have occurred when filling your prescription. It also allows you an opportunity to ask questions about your medicine and to make sure you understand the directions for taking it. Your pharmacist is a helpful source of information and can give valuable advice about prescriptions, herbals, and over-the-counter medicines.

Alternative and complementary care, also commonly known as complementary and alternative medicine (CAM), alternative health care or holistic medicine, includes any healing practice that does not fall within the field of conventional medicine. It is often based on historical or cultural traditions, rather than on scientific evidence. In Canada, as in other developed countries, the use of alternative health care is on the rise. Alternative and complementary health care often focuses on 'whole person health'.

The South East CCAC provides the public with a searchable data bank of all the health and social services in their communities and can be accessed at www.communitcareinfo.ca or they can call the South East CCAC and speak directly with an information and referral staff member. South East CCACs also provide case management by working with some clients and their families to assess their specific needs and develop a plan of service. Did you know? Arthritis becomes more common as you age because of the wear and tear on the cartilage between your bones and joints, but age itself does not cause arthritis. (Even young people can suffer from premature bone and disc disease.) However, no matter your age, you can help yourself by losing weight, wearing comfortable, supportive shoes and adopting a routine of low-impact exercises to help keep yourself flexible. A study found that women who exercised once every 2 weeks for 20 minutes were less likely to develop arthritis of the knee, most common in women who don’t exercise.

Some walking clubs focus on a particular type of fitness walking, however most clubs are casual local groups that gather for friendship and exercise. Our trails system provides an excellent opportunity for nature walks with friends or a solitary stroll to commune with your inner self.

Did you know? Active people are 40% less likely to have “silent strokes” which raise the risk of developing dementia, impaired mobility and major strokes. Research shows that for the elderly, exercise is a protective factor against cognitive decline. Even walking as much as three or four times a week can show improved or stable mental activity.

Talk to a dietitian if you feel you should have a better BMI.

Service Information: The Cloyne & District Historical Society and the Pioneer Museum is a not-for-profit, charitable, volunteer organization whose mandate is to preserve and record the history of the local area, to educate the public about that history and to emphasize the uniqueness of the area. The Society collects family and business histories of the surrounding area, accepts donations of artefacts, staffs the Pioneer Museum in Cloyne, maintains its displays and archive photographs, family histories, deeds and other records as they are donated. In order to generate funds for the beautification projects, Villages Beautiful organizes the Festival of Trees, held for 3 days early in December in Sharbot Lake. The group is non-partisan and decisions are made by consensus. Volunteers are always welcomed on the committee, to help with the Festival of Trees or work in the gardens. Crisis Line 24/7 Service Information: The Women’s Program provides mobile outreach for female victims of abuse who are 16 years of age or older in the catchment areas of Northern Frontenac and L&A Counties. Offices are located at Land O’Lakes Community Services in Northbrook with a satellite office at Northern Frontenac Community Services in Sharbot Lake. Services may also be provided in a variety of safe locations throughout the extensive catchment area of the program. The Land O’Lakes Women’s Program provides the following services to women: Telephone crisis support Information and referrals to victims of

current or past abuse Practical assistance such as

transportation/accompaniment to emergency shelter, police, medical, legal appointments and court appearances

Safety planning On-going individual and group counselling to

past and present victims of woman abuse.

2007. Occupancy in a Retirement Home is a tenancy regulated under the Residential

Tenancies Act, 2006 and there are no special health eligibility requirements. Tenants rent a room, suite or apartment and can contract with their landlord to provide meals and other care services for a fee.

Choosing your beneficiaries, and the amount and timing of their inheritance, is removed from your control and is governed by the Succession Law Reform Act. For common law spouses the consequences of a spouse dying without a Will can be particularly devastating. A surviving common law spouse does not have the same rights as a married spouse to share in the deceased’s property. A Will allows you to make provision for your loved ones. Don’t Delay!

Volunteering is healthy. It benefits both society and the volunteer’s own health. Seniors have a great deal of knowledge and experience to share. By volunteering, seniors become more engaged in their world and gain health benefits. Until recently, those health benefits were hard to define. Researchers have found that, compared to other people the same age, active volunteers reported improvements in mental health, physical functioning, higher self-esteem and more connectivity with other people. They also seemed to delay the onset of age-related health problems. Some other benefits include: a lower risk of depression, fewer physical limitations, less decline in health due to aging and increases in physical activity.
