WHAT IS PARKLAND

HEALTHplus?

Parkland HEALTHplus is a healthcare assistance program developed to provide quality medical care to qualified Dallas County residents. It is not a health insurance program.

Parkland Healthplus is good at Parkland Health & Hospital System facilities only!

HOW DO I QUALIFY?

To apply for Parkland HEALTHplus, please bring the following information to Parkland or any of our neighborhood health centers for eligibility determinations. Note: Access to health insurance may affect your eligibility.

Proof of:

• Dallas County Residency

• Income

• Identification

• Number of people in your family

WHAT IS MY COST?

There is no enrollment cost

You will be asked for a co-payment for clinic visits, pharmacy prescriptions, supply items, or specific procedures. Co-payments are due at the time you get your health services.

SERVICES AVAILABLE TO YOU

As a Parkland HEALTHplus patient, you will be able to receive medical services that are medically necessary and are normally provided at any Parkland facility. Note: Services not medically necessary may not be covered by Parkland HEALTHplus.

Only Parkland Health & Hospital System charges can be applied to your Parkland HEALTHplus account. You will be personally responsible for charges incurred outside the Parkland Health & Hospital System.

Think of your Primary Care Provider (PCP) as a partner who helps manage your medical care and well being. Parkland HEALTHplus will provide you regular access to quality medical care. Better allocation of health care resources allows Parkland to give more patients quality care for the same health care dollar.
