Time On Site: 6:35 am

6:35 am Four air monitoring stations are being setup at perimeter locations specified by the CM, forming a quadrangle around the anticipated work area. According to EnTech, air monitoring is occurring at the four air monitoring stations beginning at 6:35 am. The air monitoring station locations are depicted on the Sketch.

6:35 am Concrete pump truck on site in the southeastern portion of the site.

6:35 am to 3:00 pm Presco bending rebar in the northwestern portion of the site and installing rebar in the central portion of the site.

7:00 am to 7:45 am Four trucks enter the site through the gate and the Stabilized Construction Entrance on 25th Avenue. The GC utilizes the excavator to load the soil from the northeast portion of the site onto the trucks. The trailers are loaded with soil, raise the tarp over the trailers and exit the site through the Stabilized Construction Entrance and the gate on 25th

Avenue. According to the CM, the soil is being transported as “Material meeting PA Clean Fill Standard”. According to the CM, the soil (obtained from Waste Characterization [WC] Grid Number 35 [according to the GC-prepared manifests; Attachment A]) is being transported off-site for final disposal at: Total Recycling Corp. in Danielsville, Pennsylvania (TRC) via Impact Reuse & Recovery Center in Lyndhurst, New Jersey (IRRC). Details are provided on the GC-prepared manifests (Attachment A).

7:00 am Portion(s) of the silt fence along the southern perimeter of the site observed to be down to accommodate work activities in the area. The CM was notified. See “Issues discussed this date with the Construction Managers”.

7:00 am to 3:00 pm Box forms constructed in the southeastern and southern portions of the site. High density foam being installed in the southern portion of the site.

7:00 am to 3:00 pm Presco installing rebar in the southern and eastern portions of the site. Crane and lull utilized to move rebar around the site.

7:00 am to 3:00 pm Olympic Plumbing installing drainage plumbing in the box forms in the central portion of the site.

7:00 am to 3:00 pm Solar Inc. installing sleeves and conduit in the pile caps.

7:00 am to 2:45 pm Concrete trucks enter the site through the gate and the Stabilized Construction Entrance on 25th Avenue. Trucks poured concrete into concrete pump truck.

7:30 am to 3:20 pm Soil Solutions utilizing welding tools to weld braces inside the 36-inch piles in the eastern portion of the site.

7:20 am to 3:50 pm Concrete pump truck pouring concrete in the southern and central portions of the site.

8:00 am to 3:15 pm JT Cleary dredging Gravesend Bay in the southeastern portion of the site. Containment booms in place around the work area.

8:00 am to 3:00 pm Box forms taken apart in the southern portion of the site.

8:30 am to 9:30 am Trailer with high density foam blocks enters the site through the gate and the Stabilized Construction Entrance on 25th Avenue. Foam unloaded in the southwestern portion of the site.

9:40 am to 10:00 am & 12:00 pm to 12:30 pm

Site work stops for a work break.

10:40 am to 11:00

Three trucks enter the site through the gate and the Stabilized Construction Entrance on 25th Avenue. The GC utilizes the excavator to load the soil from the northeast portion of the site onto the trucks. The trailers are loaded with soil, raise the tarp over the trailers and exit the site through the Stabilized Construction Entrance and the gate on 25th Avenue. 4:00 pm Site work stops for the day. EnTech removes the air monitoring stations.

General Observations:

According to EnTech, particulate matter and total organic vapors were not detected at concentrations greater than the action levels.

According to Louis Berger, at approximately 6:00 am Presco began installing bending rebar in the northwestern portion of the site.

Throughout the day, the GC periodically implemented dust suppression at the site by wetting down the ground surface with water from the water tank truck. The water tank truck was observed wetting down the ground surface at an approximate frequency of once every two hours.

Issues discussed this date with Construction Manager: Silt fence along the southern perimeter of the site observed to be taken down to

accommodate work activities. The silt fence along the perimeter of the site is not observed or observed to be in disrepair.

Status: The reinforced silt fence along the perimeter of the site is not installed / located as per drawing C-013.02, titled Final Site

Grading, Paving and Landscaping Overall Plan, Construction Phase, prepared by Greeley and Hansen, dated September 2012

(revision date) and drawing 1-6, titled Proposed Stormwater Drainage Plan, Final Site Finishes Phase, prepared by Greeley and

Hansen, dated February 2013 (both included in the Project SMP) or was observed to be in disrepair. According to the CM, the silt

fence along the southern portion of the site will not be erected/restored because the concrete wall that is currently being constructed

in the southern portion of the site will extend from the ground surface to an elevation that is higher than the top of the silt fence and

rendering the silt fence useless. The CM has requested the GC review the current plan(s) regarding placement of the silt fence and

adjust the plans, as needed.
