Medical Paperwork Physical must occur within six months of initial
practice. Make sure ALL sections of the physical form are filled out. You may COPY for the health center
Medical History Questionnaire must be completed –questions related more to athletic injuries, etc Some teams required to do concussion testing upon

The NCAA conducts random drug testing each year on campus and possibly at championships
All Student-Athletes required to submit to testing or lose eligibility
ADHD medical exception: MUST provide significant documentation for
an exception. Students allowed to participate while taking
this medication, BUT should begin to acquire the information needed for an exception.
COACH Is aware of all requirements of student-athletes Will be able to assist with sport-specific issues
