GRANT INFORMATION

The Missouri Department of Elementary and Secondary Education and the Transition Coalition

partnered to support the formation of sixteen Community Transition Teams across Missouri. The

Kirksville Community Transition Team was selected for their commitment to the improvement of post‐

secondary outcomes for systemic change approach for students with disabilities.

developmental disabilities.

Eligibility: Service coordination and funding – initial eligibility must be determined

by MO Division of Developmental Disabilities (Kirksville Satelite Regional

Office); funding is based upon need, availability of funds and county of

residence. We serve residents of Adair, Putnam, Schuyler, Scotland and

Sullivan Counties.

Cost: Seldom any cost to individuals. Division of DD requires standard means

test for service coordination and funding eligibility.

If you need: Educational and resource information covering a variety of materials to

include but not limited to Business and Workforce, Children and Teens,

Cost: Classroom services are free. GED testing is done at approved testing

centers. Total cost for GED test in Missouri is $40. Application fee of

$35 for Career Training Programs.

Demand‐Response System: $2.00 per one way trip anywhere inside the

Kirksville city limits; $3.00 per one‐way trip outside of Kirksville city

limits within a 15 mile radius of downtown Kirksville. Children under 12

If you need: Assistance with employment opportunities to include Career

Development, obtaining, and maintaining employment in community

businesses, assistance in achieving community inclusion or assistance in

developing independent living skills.

Hours: M‐F 8:00 to 4:30. Once services begin services are delivered based on

the needs of each individual.

Eligibility: Individuals entering the employment program must be at least 16 years

of age and have a legal status to work in the United States. Individuals

entering the Community Skills Program should have referral from a

partnering agency.

Cost: Cost for services vary depending on the service provided. Typically

services are funded by Vocational Rehabilitation, Department of Mental

Health, or Local Senate Bill 40 Boards. A fee for service can be provided

upon request.

trips for medical, shopping, essential business, senior center visits and

employment. Weekly Express provides transportation to Columbia and

St. Louis. If you have a transportation need that cannot be met by the

local Kirk‐Tran option then contact the number above for further

assistance.

Hours: Call Monday thru Friday 8:00am – 3:00pm to schedule a ride. You need

to contact the office at least 24 hours prior for a reservation for the

Weekly Express. If you need to make special arrangements then contact

the office at least one week prior to the day you need service.

Eligibility: OATS transportation is available to anyone regardless of age, income,

disability, race, gender, religion, or national origin.

Cost: Fares are $10 for a round trip; $6 for a one‐way trip. There may be

funding to cover the fare for people who are 60 years of age or older,

with disabilities (age18‐59), or Medicaid eligible going to a Medicaid

provider.

for your rehabilitation. We will give you vocational information and
