I seem to be treating a run of broken bones at the moment. These vary between patella (knee caps), ankles (tibia and fibula) and head of humerus (shoulder joint). The diagnosis of ‘fracture’ or ‘break’ always confuses people so I thought I could discuss the general principles of these types of trauma!

A ‘break’ or a ‘fracture’ is essentially the same thing. In this article I will now use the medical term of fnow use the medical term of fracture. There are many types of fracture. The first is a ‘Greenstick’ fracture. This is an incomplete break of a bone- ie the two bits are not separated. These are common in children who fall on their wrist- indeed I had a greenstick fracture of my wrist (from memory the radius bone) following a roller-skate disco injury aged about 13! Bone in children is not fully calcified until they are around 18 years old so the soft structure is like a bendy young soft twig on a tree! It tends to be put in a splint and splint and recovery is swift after 4-6 weeks.

A comminuted fA comminuted fracture is where there are several bits of bone. They are seen in fractured collarbones when you have fallen onto it, or a direct blow to the kneecap from a fall. Whilst not good to look at on X-ray these often heal well as there is lots of surface area to provide healing activity. Wiring can be used however to help piece it back together…! Conversely, a transverse fracture of a long bone such as the femur (thigh bone) means a break straight across in a horizontal line. These may well need pinning to stabilise, whereas an oblique or spiral fracture- the break runs down through the bone as the words suggest, again, often have better surface area for healing.

A ‘compound’ or ‘open’ fA ‘compound’ or ‘open’ fracture is less than pretty… the bone has pierced its way out through the skin- definitely time for rapid hospital as the risk of infection is very high. I dealt with a nasty one of these at a volleyball match not long ago…. Ooooh I remember that well!

Another common fAnother common fracture I see, often in ankles or shoulders is an ‘avulsion’ fracture. This is where following a twist in an ankle or a dislocation in the shoulder joint, a small portion of bone at an attachment site for a tendon or a ligament, literally pulls a piece of bone away. Sometimes these will need fixing surgically but often rest and immobilisation of the joint will allow natural healing to occur. Last but not least in the frequent fracture list is a ‘crush’ fracture. These can be from impact- a classic is landing on your heels having fallen from a height, or also a spinal crush fracture as a result of osteoporosis. The bone is simple crushed or squashed.

TThe amazing thing is, the body has a fabulous ability to heal! So, splinting, surgery, slings, casts and rest will allow the body to do its stuff. Once the bone is deemed stabile, physiotherapy can restore the range of movement and strength to the affected area…and on we go with our lives again!!

The battering of rain upon her windowwoke Helen from an unpleasant night thathad delivered a terrifying nightmare. Adream so twisted that she could still recall it.The company she worked for had decidedthere would no longer be any desks orchairs. They would have to hang upsidedown from the ceiling like bats only comingdown at the end of the day. Also, the newman in charge had confiscated the officekettle; tea breaks were to be a thing of thepast. It was a crazy dream.

Helen had been awake for hours lyingnext to her partner Paul who was probablydreaming of his precious garden and notabout her. He turned over every so oftenshaking the foundation of the bed asshe hung onto the duvet for dear life. Hisenormous body was demanding morespace as she waited for the small snort healways made as he momentarily woke, thenreturned to his flowerpot world.

She was dreading the thought of goinginto work and seeing the new boss whodisagreed with the way she did her job. It

had been much more fun before he becamesales manager.

She glanced at the clock that still hadthirty minutes before the alarm wouldsound and decided she’d had enough ofher restless sleep. She slipped out of bedinto the cold air only to curse Paul forinsisting the heating remained on low inthe mornings. He’d say, ‘Turn it down justone-degree Helen and think of themoney we’llsave’. Only, she was really finding it too cold.It was alright for him, retired and no rush toget up.

Down in the kitchen Helen reachedfor the thermostat twisting the dial to amassive thirty-five degrees. Paul could roastin bed for all she cared. There was no wayshe was going to put up with the coldany longer.

She turned her attention to the cat whowas fast asleep. Lifting Harry from the warmblanket, his claws clung to her dressinggown sleeve. Easing the back door openHelen changed her mind; it was far too coldand wet to put their pet out.

Helen braved the awful weather to driveto work and arrived fifteen minutes early.She was determined to get her act togetherand show Jonathon Medley she couldadjust to the changes. Having raided thecleaner’s cupboard for furniture spray and acloth, in no time at all she had the smartestand best smelling desk in the company.

When her colleagues arrived, their eyeswere immediately drawn to the sparklingwork station, and more importantly toHelen being in the office so bright and early.

In came Jonathon in his smart pinstripesuit and floppy red hanky. He smiled at thegirls, his good looks bowling them over – orso he thought. He queried where the strongof smell of polish was coming from. One ofthe girls pointed to Helen’s gleaming desk.Raising his eyes to the ceiling he askedHelen to follow him to his office.

Inside his glass sanctuary with the newcarpet and blinds, he waited for her to sitdown before perching on his swivel chair.He ran fingers over the sharp creases ofhis trousers and removed a piece of tissuefrom the fabric. Holding it at head height heblinked a number of times.

‘Give me one good reason why Ishouldn’t fire you? It will take more than abit of polish to convince me you’re pullingyour weight.’

She sighed, ‘I’m working as hard as I can.What do you want me to do? Shall I hangfrom the ceiling like a bat whilst I make mycalls?Would that please you?’

‘See what I mean Helen, this is just a joketo you and you’re hardly ever here.’

‘That’s not fair. I can’t help it if I’m sick.’

She pretended to sniff, ‘I’ve got a coldtoday and I’ve come in.’

‘Doesn’t sound like much of a cold.’

Jonathon screwed up his face, ‘Okay thenlet’s go over last week’s territory alignmentfor the new representative. It’s so badlyplanned, Helen.What were you playing at?’

Helen felt there was only one thingto do and apologised saying she got thegeography wrong.

With her head hung low she hopedhe’d think she was upset. If the truth beknown Jonathon had been a great loverand colleague before he got the promotion,but recently she couldn’t do anything right.Making sure he saw her, she rummagedthrough her handbag for a tissue.

‘And you can cut out the waterworks.’

She wasn’t shedding any tears (quite theopposite). This morning’s episode with herboss had helped her to see clearly; she nolonger needed the job.

Jonathon rose from his desk and turnedto look out of the window. He then asked,‘Have you considered working in oursubscriptions department? You’d do betterwith that than sales?’

He continued standing with his back toher and kept on talking. However, she waslong gone skipping down the corridor backto her desk. She took delight in emptyingthe contents of her drawer onto the floor.

The entire sales team looked up, andone of the girls lowered her headset to say,‘What’s up, Helen?’

Helen sarcastically replied, ‘Just having aspring clean – you should try it sometime.’

She drove home in bright sunshine,feeling refreshed. Partner, Paul, would gethis marching orders next as the spring cleanof her life continued.

There was no room for two pathetic menin her life.

A Co-op Funerals' report, Burying Tradi ons: The Changing Face of UK Funerals,found that three quarters (77%) of Co-op's funeral directors said requests for

Even before the Chancellorannounced the stamp dutyholiday on Wednesday 8July estate agents had seenunprecedented demand frompeople wanting to buy property.Certainly at Dixon Stewart June2020 was a far busier monthfor new instructions than June2019.

How does the stamp dutyholiday work? The tax thresholdhas been temporarily raiseduntil March next year to£500,000 in England andNorthern Ireland. Anyonecompleting on the purchase ofa main residence between 8July and 31 March 2021 willnot pay any stamp duty. Moreexpensive properties will onlybe taxed on their value abovethat amount. Buyers will saveas much as £15,000 if they arebuying a property of £500,000or more.

Clients who had alreadyexchanged contracts on ahouse purchase with completionfor 8 July onwards got aninstant saving on money theyhad already paid to us. The

Government hope that theywill use this money in orderto help boost the economyby purchasing something newfor their home or carrying outimprovements.

The stamp duty holiday isintended to boost a propertymarket hit by lockdown and tohelp buyers who have takena financial hit because of thecoronavirus crisis. Buyers will bein a better positon to qualify fora mortgage as reduced stampduty allows them to pay a largerdeposit. Sellers are more likelyto come to the market duringthis period when they believebuyers will have more money tospend.

There is a concern that whilethe stamp duty holiday mayboost the property market inthe short term it could also leadto a spike in sales that pushesup prices before the deadlinefollowed by a slump next spring.This was observed in 2016before the 3 per cent buy to letand second homes surchargewas introduced.

Extending the stamp dutyholiday to help buy to letpurchasers and second homepurchasers was a surprise.Investors have in the pastbeen hit by higher taxes. Beforethe stamp duty holiday theywould have paid £30,000 instamp duty on a £500,000purchase. They will now pay£15,000.

First time buyers who werepreviously exempt from payingstamp duty up to £300,000 willlose their special status duringthe holiday paying the samerates as people moving home.

The Chancellor also promisedthat from Septemberhomeowners and landlordsupgrading the energy efficiencyof their homes would be able toapply for vouchers to cover atleast two thirds of the costs ofimproving their properties up toa maximum of £5,000.

WETAKECAREOFYOURBESTFRIENDI was little more than knee-high to agrasshopper when I first discovered my love ofanimals. Hugely inspired by the tales told inJames Herriot’s “All Creatures Great andSmall”, I set about caring for pretty much anycreature that found its way into our backgarden. Badgers, hedgehogs, stray cats,beetles. You name it, it was lovingly nursedback to health in a suitably sized cardboard boxor jam jar. At seven years old I had big dreamsof becoming a vet, just like James.

I appreciate that, given the choice, most peoplewould favour a luxury retreat in the Maldivesover a week’s volunteering at an animalsanctuary. But one evening, out on a sunsetdog walk, I rediscovered the dreams of myseven-year-old self.

Dyslexia Awareness Week: 5th - 11th OctoberDyslexia Awareness Week runsfrom the 5th to 11th October and atMy Kind of Thinking we haveforged links with local bookshopsto raise awareness and further theunderstanding of dyslexia. We arealso running an online campaign tobring together resources tosupport families and young people.

Although most people have heardof dyslexia, it’s often mistakenlythought of as just a difficulty withreading and spelling. DyslexiaAwareness Week aims to foster abetter understanding of what it is,and a recognition that it affectspeople differently. For example, forsome it can be a real barrier toeducation and employment, whilefor others it may just mean milddifficulties with spelling.

What is Dyslexia?

Dyslexia is characterised bydifficulties in understanding soundsand letters, working memory,speeds of processing informationand remembering information

given out loud. It can often occuralongside other Specific LearningDifficulties (SpLDs) such asdyspraxia or ADHD.

Dyslexia does not impact on anindividual’s intelligence, and in factmany people with dyslexia havehigh levels of ability in numerousareas. It affects everyonedifferently, and one person withdyslexia may have a very differentprofile of strengths andweaknesses to another person.

For some individuals, dyslexia canimpair their ability to rememberinformation needed to perform atask; for others the primarydifficulty might be in organisationand time management, or theirability to spell and read with ease.

It is thought that dyslexia affectsaround 1 in 10 people, althoughmany adults may compensate welland work towards their strengths.It is only when they are required tocomplete more complex writtentasks that their difficulties becomeapparent.

How is dyslexia diagnosed?A dyslexia assessment provides aconfirmed diagnosis and allows aclear picture of the individual’sstrengths and weaknesses toemerge.

There is no single test that candiagnose dyslexia - a diagnosticassessment consists of a widerange of tests which explore areasincluding verbal and visual ability,memory, processing speeds andawareness of spoken sounds(phonological awareness). Inaddition, areas of attainment suchas reading, spelling, writing andcomprehension are tested. Theresults of all these tests areconsidered alongside theindividual’s background history, aswell as observations of theirbehaviours and performanceduring the assessment itself.

Dyslexia assessments are carriedout by educational psychologistsand specialist teacher assessors,who are qualified teachers withextensive postgraduate training indyslexia and other learningdifficulties. Each assessmentprovides an in-depth report, whichincludes results of the assessmentand detailed bespokerecommendations for theindividual.

One Pan Sausage, Kale and PotatoIngredients:

• 2 tablespoons of olive oil,

• 150 grams red potatoes (cut into 3/4inch wedges),

• 200 grams of spicy Italian sausage(sliced into 3/4 inch slices),

• 1/2 red onion (sliced thinly),

• 750 grams of kale (chopped),

• 1/2 teaspoon of smoked paprika,

• 1/2 teaspoon of dried oregano,

• pinch of sea salt,

• pinch of fresh ground black pepper,

• chopped fresh parsley for garnish,

• 1 lemon cut into wedges for serving.

Directions:

1. Heat oil in a large frying pan or paellapan over medium-high heat. Addsausage slices in a single layer andcook for 3 minutes until beginningto brown. Flip and continue to cookall sides until browned and cookedthrough. Remove sausage andset aside.

2. Add potato wedges in a single layerand season with salt. Allow to cookfor 3-4 minutes until browning, flipthe potatoes and cook until tender.Remove potatoes and set aside withsausages (same bowl/plate).

3. Add the red onion and cook for about5 minutes until translucent.

4. Start mixing in the kale a handful at atime, stir, and then continue to add inall kale until it all fits in your skillet.

5. Season with salt and occasionally stirfor about 3 minutes until just wilting.

6. Add back in the sausage, any juiceson the plate, and the potatoes.

7. Stir in paprika, oregano and pepper.

8. Serve with lemon wedges and parsleygarnished on top.
