Chapter-1Introduction & Executive Summary


1.1 INTRODUCTION AND GENERAL

The axis Devihalli to Hassan encompasses the road along NH-75 (Old NH-48) which is a 4 lane road except for the bypasses Hassan and Channarayapatna which are two lanes. The road traverses plain terrain generally with a traffic intensity which is moderate except at nodes/town junctions where movement is denser. Some of the townships like Udayapura (Km.155) and Shantigrama (Km.167) encountered in transit besides Devihalli and Channarayapatna are Hirisave (Km.123) and Gulsinda (Km.146). The density of population in the townships is not very heavy. The people form a mix of rural inhabitants and urban dwellers with the forte in occupation being principally agrarian. The language spoken is Kannada with a dialect in villages. Town dwellers are engaged in government pursuits with townships fairly well provisioned with educational institutions. Most of the village/town nodes are supplied with electricity and restricted water supply though at no time is water really in deficit. Devihalli is a sleepy village on the way to becoming a township in due course whereas Hassan is a developed town. Power cuts are limited. Generally water is in adequate supply to inhabitants and no grave shortage is recorded. The temperature is in the range of 400C during summer months and pleasant at nights whereas winter months are cool (170C at night). Day temperatures hover around 250C to 300C ambience on sunny days. Hassan is a gateway to mountainous stretches, beyond it towards Mangalore. The altitude of Hassan is 934 Meters above MSL (Mean Sea Level) whereas Devihalli is 778 meters above MSL (Mean Sea Level). Hassan is not industrialized. It also is not a cantonment. The administration provides a satisfactory facility for commutation by buses within town and around to outskirts and back. The Road has no zig though some expansive horizontal curves exit. The townships of Channarayapatna (which is fairly dense) and Hassan are slated to have 4 lane paved bypasses respectively on upgradation from the present 2 lane on sanction during pendency of this project. This is expected to start in November 2016. Accidents on the road are prompted basically by the carelessness of the drivers by over speeding or drunkenness which with more slogans besides the road furniture extant is expected to provide marginal relief. We are also exhorting the Concessionaire to expand/ improve the number and quality of signages. They have to be catchier. Some locales worth visiting around Hassan are Chickamangalur, Bellur and Hallebidu. Shravanabelagola (a famous Jain Pilgrimage centre) is situated on a lateral 30 Kms from Devihalli and it is easier to go from Channarayapatna. The 4 laning of Devihalli to Hassan except bypasses was almost completed during October end 2015 but Plaza works which are incomplete and a few civil works (Law and order problem) are being hastened. TP1 is under a penalty and for 2 months work is stopped. TP2 is proceeding at a snail’s pace and also under penalty. We had recommended penalty to be levied Vide our letter No 1004/MSV/D-H/IE/TL/Tech/(Plaza)/19:date:02-May-2016 . The Concessionaire must rise up to the occasion and finish the pending works earliest to avoid complications.

The Mahamastabhisheka of Gomateshwara (Eldest son of the first Jain Tirthankara Rishaba) at Shravanabelagola on a lateral 18 Kms off Channarayapatna is scheduled in 2018 (once in 12 years) inviting a huge surge of pilgrims, other state visitors and foreigners. This is performed with great austerity. The roads shall have to be top class with drainage in proper form. Efforts towards this end are at hand. Signages should be self revealing and catchy.

1.2 EXECUTIVE SUMMARY

The Devihalli-Hassan project road traverses from Km 110+000 to Km 189+500 between Devihalli to Hassan on NH-48/75 in the state of Karnataka. The project was awarded to M/s L&T Devihalli-Hassan Toll Way Ltd. on 30 April 2010 under DBFOT pattern. The project construction period is 30 months and the concession period is 30 years. The project commenced on 14 December 2010.

As of now about 69 months have elapsed. The cumulative physical progress for the month of September 2016 is 68.48% (80.52% for TP1 and 56.35% for TP2) including Plaza compound walls and environment works (excluding PCOD) Financial progress 2.28 Crores (out off 3.7 Crores). The reported cumulative overall physical progress (Plazas and Civil works) including environmental Work by the end of September 2016 was 99.53%.

In respect of timelines to be met by the Concessionaire, Project Milestone-I, II & III have been achieved within stipulated time frame in the project. Schedule two/four laning date had been due by 10 June 2013 (910th day from the appointed date).EOT-1 has been approved by the Authority up to 31/12/2013. The Concessionaire had also applied for EOT-2 for completion of COS-II works which has been reviewed and recommended by the IE up to 31/10/2014. The NHAI had approved the EOT-2 up to 31/10/2014.The Concessionaire had also applied for EOT-3 up to 30/09/2015 for completion of COS-II works which has been reviewed and recommended by the IE up to 31/07/2015 and extended up to Feb 14 -2016 (EOT-4). This has lapsed. He has again applied for (EOT-5) till September 2016 and IE has recommended EOT-5 (for Plaza 2) up to June 30, 2016 with the provision; it takes effect on the performance. If he fails to perform he invites liquidated damages. This extension was later kept under suspension and now cancelled for no progress. A penalty on Plaza 2 has been recommended for levy vide our letter No 1004/MSV/D-H/IE/TL/Tech/(Plaza)/19:date:2-May-2016. It may be recalled the previous IE had already levied a penalty on Plaza-1.

Provisional Completion Certificate-I had been issued for 59.7 Km on 14/11/2013. Provisional Completion Certificate-II had been issued for 14.652 Km on 05/12/2014. Provisional Completion Certificate-III has been issued for 2.876 Km on 06/10/2015 by the IE. The total Project Highway of 77.228 Km (100%) has been completed except some line drains and service roads hindered by law and Order problems attempted to be surmounted and lingering for long. Toll collection is in progress at Toll Plazas 1 & 2. Toll building construction is under progress. The balance of civil works PCOD and Plazas including the Drain, Service Road and Metal Beam Crash Barrier amounts is 3.45 in Crores.

No extension is currently in vogue after the last expiry on Feb 14, 2016.Routine site inspection is carried out by Independent Engineer’s site staff on day to

day basis for ongoing works. Tests on material are conducted by the Independent Engineer if found warranted. Besides, pursuant to Schedule-Q of TOR, site inspection had been conducted by Team Leader on September 30, 2016 for the entire stretch of the project road. Inspections are carried out jointly by the team of Independent Engineer headed by The Team Leader, Key personnel and Concessionaire’s representative besides exclusive IE’s inspection Please see Annexure-A . The inspection starts at Km 110+000 and proceeds towards the end chainage up to Hassan. In general all site activities have been observed; however, close site inspection has been made at representative locations and remarks given. Monthly inspection summary carried out during the month of September 2016 is enclosed.

All design issues in the project such as revised pavement design, revised plan profile drawings, typical drawings of project facilities and design drawings of structures had already been completed and necessary concurrence for them accorded. Changes of Scope Notice for



additional structures/supplementary work when issued were executed and completed integrating the same to the original contract.

Construction of major structures in the project such as Minor Bridges, Box Culverts and Pipe Culverts had already been completed. All 66 pipe culverts 26 additional pipe culverts completed, 14 of Box culverts and 21 of Minor Bridges have been completed.

Recommendation along with Design, Drawings & Cost Estimate for COS I, II, III & IV were submitted to NHAI for issue of Change of Scope Order. NHAI have issued the Change of Scope Order vide letter No.NHAI/22015/1/10/PIU-BNG/1588 Date/08/07/2014 for an amount of Rs 17.75 Crores. As on date the payment made is Rs 16.28 Crores. Design and Supervision charges have not been admitted as not permissible.

Road Signages require to be reinforced as these had been inadequately provided. In the interest of commuters and for wholesomeness we are in the process of recommending additions under Change of Scope.

The riding surface is in general good. Where cracks were discernible at the tail end an overlay has been laid. The surface shows distress again towards the end see appendix ‘A’ and action is contemplated to smoothen the riding surface. We feel the latter traverse of riding surface of this Project (4-Lane prior to Hassan Bypass) is qualitatively inferior compared to the rest of the Project which is satisfactory. We are investigating. Rectification are in Progress.




Chapter-2The Project


2.1 BackgroundThe Government of India has entrusted to the National Highways Authority of India (NHAI),

under Ministry of Shipping, Road Transport and Highways (MORTH) for the development of highways under National Highways Development Program (NHDP). The projects contemplate enhancing the traffic capacity and safety on the selected heavily trafficked high density stretches/corridors of National highways for efficient transportation of public and goods.

The Project Corridor under consideration falls under NHDP Phase III A program of 2/4 laning to be undertaken on Design, Built, Finance, Operate and Transfer (DBFOT) basis.

The Development of Project consists of four laning of Devihalli-Hassan Section of NH-48/75 from Km 110.000 to Km 189.500 including 2-lane with Paved Shoulders Bye pass for Channarayapatna Town and widening of existing Hassan Bye pass to 2 lanes with Paved Shoulder in the State of Karnataka.

2.2 Project BriefThe highway project described in this report runs from Km 110.000 to Km 189.500 between

Devihalli to Hassan on NH 48/75 in the state of Karnataka.The project was awarded to Concessionaire M/s L&T Devihalli Hassan Toll way Ltd and

LOA issued on 30th April, 2010. The Concession Agreement was signed between National Highways Authority of India & M/s L&T Devihalli Hassan Toll way Ltd .on June17, 2010.

The appointment date is December 14, 2010 and completion date is June 10, 2013. Extended completion Date is Dec 31, 2013.PCOD III final date is Oct 06, 2015 which has lapsed.

LEA Associates South Asia Pvt Ltd. had been appointed as Independent Engineer (IE) of the project. Independent Engineer mobilized since May 09, 2011. The IE had established their office at Channarayapatna town which is located about middle of the project highway and all of the key professionals & sub professionals staff had been mobilized.

MSV International, INC. has been appointed as Independent Engineer (IE) of the project and mobilized on 20th OCT 2015.The new IE for O & M and construction of 2 Lane bypasses is based at Hassan with full strength presently. Key staffs have been mobilized.

2.6 Scope of Work

The design specification for the Devihalli-Hassan Section is as follows:-

Four laning from Km 110.00 to Km 189.50, provided that in the following stretches, the width of the carriageway shall be Two lane with paved shoulder without a median for Channarayapatna bypass(New) from Km 139.677 to Km 148.824 and Hassan bypass from Km 175.163 to Km 184.912.

1. 21numbers of Minor Bridges, 14 numbers of Box culverts and 66 numbers of Pipe culverts on main carriage way are have been widened /reconstruct as per Schedule B of Concession Agreement.

2. There is 1 Box Culvert and 8 Pipe Culverts which are not available at site and these were proposed under negative Change of Scope and deleted.

3. Change of Scope notice for 3 VUPs, 1 Minor Bridge, 6 Box Culverts and 26 Pipe Culverts and service road has been issued by the Authority and work executed.

4. ROB/RUB, Grade separators, Pedestrian/Cattle Underpasses& Service Road have NOT been proposed for the project highway. Service Road came under Change of Scope.

5. Traffic Management and Safety of workers and the Public shall be maintained at all times in accordance with the relevant IRC/IS Codes as stipulated in the Concession Agreement without any deviations.The scope of the work for the Operation & Maintenance of Project Highway:-

Road side furniture Pedestrian Facilities Highway Lighting Systems Junctions(Major & Minor) Tree plantation and landscaping Truck Lay byes & Bus bays with bus shelters Four laning of Channarayapatna and Hassan Bypasses.

Except for the last point as above rest stands almost completed with the Operation and Maintenance Phase now on.

2.11.1 Organization Set-up Authority

In terms of Concession Agreement, National Highways Authority of India is working as Authority. Project Implementation Unit at Bangalore is headed by Project Director with Manager Technical. Regional Office is headed by Chief General Manager.

Independent Engineer

Independent Engineer took charge of duty on 20th OCT, 2015. The team at site is headed by The Team Leader. All key staff members have been mobilized. Part Time/ Intermittent staffs are on as is required basis. Reviewing Design, Drawings and submitting a assessments to NHAI offices as relevant is on the instructions of Team Leader. In addition to above, Team leader as the functional Head, coordinates with his Team & relays and instructs necessary inputs of a corrective nature to Concessionaire. He keeps Project Director informed as an implementation advisor. Table 2.11.2 shows the deployment of IE.

Chapter-5Quality Assurance/Control


5.1 Quality Certification

As per the provision of Schedule-Q of CA to check adherence to quality and confirmation to specification and standard laid down in the Concession Agreement, Independent Engineer shall require the Concessionaire to carry out or cause to be carried out tests on sample basis. The sample size of the tests to be specified by the IE shall comprise 10% of the total quantity or no. of tests prescribed for each category.

The Independent Engineer had been adopting the above guideline and performing tests and inspections in the project is found necessary. However, the remaining number of tests and inspections are carried out by the QA/QC cell of the Concessionaire. Based upon the test results, inspections and other items in the check-list, approval of works performed during the month is accorded. If any road stretch or any part of the works fails to comply with the requirements of density, levels, cube strengths etc., then such stretch of road or part of the work stands disapproved and NCR was issued. For such instances, remedial actions are taken and new RFIs are raised for inspection in the case where remedial action cannot rectify the deficiencies then such works are removed from the project site.

Further, we would like to state that, part of the works completed during the month which have been approved vide respective RFIs with supporting documents are certified for adherence to quality and compliance to project technical specification, and standards.

Quality Assurance: A detailed project Quality Plan has been prepared with detailed construction

activities methodology and Assurance Plane with Acceptance Criteria. IRC: SP 57-2000 and IRC: SP 47-1998 have been followed for Quality assurance of Q3 standards. Pertaining to IRC: SP 84-2009 of CA,QAM covering quality system and QAP are implemented in project work.

Note: All the above were during Construction Period now during O&M Laboratory is maintained on minimum requirement and random tests are carried out. Some equipment is yet to come to ensure minimum functionability. A time limit till 12-10-2016 has been laid out before initiating penal action for inadequacies in terms of the Contract.



6.1 O&M Inspection Report

The inspection started from the start chainage Km 110+000 and proceeded towards the end chainage Km 189+500. In general all site activities have been observed .However, close site inspection has been made for O&M works of PCOD- I, II & III stretch of 77.228 Kms. Observation are made as below:

6.3 TL’s Inspection on dated 12-7-2016 along with Highway Maintenance Engineers.



Team Leader accompanied by both Highway Maintenance Engineers inspected the Sector and the following composite observations are recorded.a) Pothole which has appeared at Km 181+200 (RHS) should be repaired as per norms without delay leading to aggravation.

b) MBCBs which are dented and damaged at various locations need to be under a scheduled programme be replaced. (The MBCB Sections are slotted with a release cushion (gap) for elastic resistance on impact. While repairing sections shall be replaced as such retaining the cushioning rather than ruining the cushion effect by short term undesirable methods as welding which due to metallurgical differential expansion could strain and distort the elements besides introducing rigidity which will keep the elements of the MBCB in severe tension and liable to impact breakage). A new damage is observed at km 179+150, Km 165+150 and Km 142+800 (RHS).

c) There should be a lined drain cleaning scheme to be published. Line drains are getting blocked. Cleaning and desiltation for part of premonsoon and post monsoon activity and should be regularly attended to. One time cleaning will not do. Line drain water stagnation at Km 171+350 (RHS) and Km 128+700 (RHS) shall among others be attended.d) A programme shall be instituted and intimated to us for the removal of ROW encroached boards. A new board has come up at Km 165+800(RHS) of Vishwas Restaurant and at Km 123+600(LHS). These shall be removed.

e) A geometrically wrong sign board at Km 120+830 (LHS) shall be removed.

f) The Hospital Sign Board at Km 123+600 (RHS) shall be properly fixed.g) Wrong figurative board shown for speed breaker at Km 123+000 LHS and shall be appropriately replaced. h) The arches of the stones at Km 131, 151, 152, 153, 171 and 172 are not painted in Golden Yellow. The rise of some these stones at these above locations need to be corrected while painting.i) Erect the Delineator which has fallen down at Km 157+970(LHS).j) All Median Blinkers not functioning shall be repaired including the recent one at Nuggehalli junction.k) All lanes of Plazas must be rendered open at all times unless on repair.l) In earlier observations, we desired replacement of defective studs in site; this has not been done including the latest ones at Km 162+900 (RHS) and at Km 164+050 (RHS). The non-repair and replacement have a bearing on safety.m) The apparent structural disturbance of the direct piers resting beams on sheet bearing on the Rail Cum Road Bridge at Km 179+100 shall be monitored from underneath when heavy vehicles pass. Any deterioration further or increase in vibrational noise shall be reported. Railway Engineer has been contacted for a joint visit and repairs as needed. The transverse cracks on approaches and along bituminous wearing course on deck transversely at expansion joint sections caused by movement vibrations shall be investigated and repaired after a joint visit. The parapets of the approach Return have sunk, which shall have to be investigated and corrective action of consolidation done. n) Head walls of culverts and parapets of bridges if broken shall be repaired.



o) The Hassan Byepass Road Marking with correct Thermoplastic Paint should resume in fair weather. We had stopped the work which was being done with wrong material.p) Avenue planting is not seen.q) Fill the gaps of median plants especially at Km 125+400, Km 160 to 163, Km 158, Km 151, Km 139 to Km 140, Km 130 to Km 131 and Km 153+580. r) 50% of High Mast lighting on TP2 is not working still. TP1 is working presently. s) Repair damaged bus stops at Annenahalli (Km 156+400) LHS.t) Pitching slope joints of Hirisave VUP on both sides are overgrown with small trees and bushes which if not uprooted and cleaned will displace the stones.u) At Km 151+800 an unauthorised opening is closed but finishing shall be soon done. So also at Km 135+800, Km 137+100, Km 137+700 and km 164. v) At major accident site at Km 177, your Ambulance driver was moving among crowds without an uniform. This is improper. People should be in uniform.w) Patrolling vehicles must be more effective in reporting incidents.x) Acrylic Enamel shall be used at Kerbs henceforth. Poor quality paint is peeling off / getting faded at many places as Km 164 RHS, Km 162 RHS, Km 160 RHS, Km 156 RHS, Km 127 RHS and Km 152 RHS.

These locations shall be specially attended.y) Rumbler painting must start forthwith to avoid accidents.

6.4 Team Leader’s Joint Inspection (31.08.2016).

The Team Leader noticed the following points in special:-a) Encroachments are on the rise. Due priority does not appear to be accorded. The

concerned officer to monitor this aspect is also offitiating as TP1 Manager and should at the earliest be shifted to his earlier post to do justice. TP1 Manager must be selected soon. Encroachments must be nipped in the bud.

b) The stretch km 176+000 to Km 175+000 (RHS) has alligator and longitudinal cracks greater than 3 mm and shall be raked out and an overlay provided. Failure of the wearing coat observed here is premature and must be investigated.

c) Road and Kerb markings are not made with approved paint. Frequent fadings cause redoing of the work resulting in waste of effort and expenditure. We recommend Acrylic Enamel for kerbs to last longer. Manufacturing Test Reports of Thermo Plastic Paint prior to use must be invariably handed over to Independent Engineer.

d) Stagnation of water on carriageway should not normally happen. One reason is spill over from jammed drains. A regular pre-monsoon / monsoon clearance of drains must be scheduled in the interest of unhindered movement. Berms should follow the one side camber of the carriageway to exit the runoff.

e) Essential and few Horizontal curve / S curve boards have still not been installed. This shall be done forthwith.

f) The 2 Railway Bridges are not in very good shape. Joint inspection shall at the earliest be arranged.

We have to impress upon the urgency to the Railway Officials. Cut roads regradation must commence earliest.

Composite points viz. Appendix A to B shall be attended to before the next inspection.

Please adhere to the timings in Appendix-1 as assured.

6.5 Team Leader’s Joint Inspection (30.09.2016).

The Team Leader observed the following points in special and an assurance was given by the Concessionaire that rectifications will be effected:

1. Lane marking with Thermoplastic painting- By NOV 1st week, work must start.

2. Kerb painting with acrylic enamel paint - By NOV 1st week, work must start.

3. Junction marking is not visible at chainages at Km 182+250 (Gorur Junction) and Km

180+000 (Mysore Junction) By NOV 1st week, work must proceed to clear this.

4. All Rumble strips painting- By NOV 1st week, work must start.

5. Contact Numbers on Ambulance- Noted for compliance by Concessionaire.

6. BC overlay warranted at km 175+000 to 174+000 (RHS)- Noted for compliance by

Concessionaire.

7. Encroachments: Hotel name board of Adyar Bhavan, Nisagara at km 153+800 LHS) and

Rajat Daba at km 175+100 (RHS)- The details to Authority in prescribed form with lodged

FIR reports shall be sent and effort shall be made to remove the boards with police support as

needed.

8. At km 166+200 (Shanthigrama Service Road), work stopped after earth work. Concessionaire

averred that as payment was pending the owner Mr. Krishna was obstructing the line drain work-

Team Leader advised the Concessionaire to check up with the dependent Tahsildar and SLAO

for their action as relevant. Team Leader shall be present when they come to site to sort out this

issue.

9. At km 167+200 (Santhigrama), RHS pipe line shifting work is in progress.

10. Silted and damaged wall repairs of Rain Harvesting structures should be attended

immediately.

11. Lighting at Bus bays, Truck Laybyes and Median Blinkers shall improve.



12. Rectification of Cracks not as per standard emulsion treatment. Cracks more than 3mm have

reflective cracks perceived after emulsion treatment. The affected stretches shall be redone /

scarified and overlaid. If cracks are extensive for 3 mm and below and incase cracks are reflected

on capped emulsion. That stretch shall be relaid then.

Cracks if visible even if capped with emulsion shall be redone interms of Clause Schedule ‘K’

of the Contract and Clause Chapter 3 (Pavement) of the O&M Manual, at km 108+800 to

108+600.

13. Lodged complaints at Channarayapatna byepass at KM 144+450 shall be reported to

Authority in prescribed format. IE shall be informed.

14. At km 123+100 Land acquisition at Hirisave (RHS) of Sakamma is now in court. Service

Road construction is affected.

15. At km 168+000 to 169+00, the median plant soil should be replaced as plants are not

flowering. Mere manuring does not help.

16. At both Toll Plazas as centre line marking is faded; painting should be done with the

thermoplastic paint as per specification.

17. At km 131+000, truncated hazard marker should be replaced with new hazard marker.

18. At some locations earthen shoulders are eroded. The correct profile should be restored and

consolidated.

19. At both Toll Plazas Civil work is stopped due to Kaveri dispute. It is time work starts. Please

peruse my letter No. 1004/MSV/D-H/IE/TL/Tech(WORK)/61 dated 24-09-2016.(copy attached)

20. At Barguru junction High mast crown light is tilted. Needs to be straightened in position to

be plumb.

Chapter-7Traffic & Safety Measures


7.1 TRAFFIC & SAFETY MEASURES

1. Traffic control & safety devices viz., Signs, Delineators, Blinkers, Barriers, etc., at the work sites as per relevant standards viz. NHAI-Technical Specification, IRC: SP-55-2014 & IRC: SP: 84-2014 have not been provided by the Concessionaire in full.

2. All Working Zones must be provided with adequate lateral & longitudinal buffer clearances as per IRC: SP: 55-2014.

3. Lapses observed at site in providing necessary PPE viz. Safety Shoe, Gloves, Jackets, to the workers. Necessary safety jackets, gloves, shall be provided to all workers at site by the Concessionaire.

4. The Concessionaire has been asked to comply whenever the deficiencies observed in the traffic safety measures have been pointed out. Full compliance awaited.

Narrative Report

Pre-construction activities includes land acquisition, encroachment removal, relocation of religious structures, utilities shifting, shifting of tank bund and cutting trees from right of way.

Attention has been paid for removal of hindrances. Recently some more areas have been cleared with the help of NHAI representative and SLAO. Now the hindrance affected length at end of Sep 2015 has reduced on both LHS and RHS of the Project Highway

1. 100% of land was handed over to the Concessionaire by NHAI.2. 98% of Electrical lines shifting works has been completed.2% of electrical shifting is yet

to be completed in Mandya district and Hassan district.3. 99% of Water line shifting works has been completed.1% of water line shifting is yet to

be completed in Mandya district and Hassan district.4. Additional land for Toll Plaza 1& 2 handed over to the Concessionaire by the Authority.

Conclusion



The 3 months ending on Sept 30, 2016, have not given any encouraging sign. Progress is almost nil at Plazas and PCOD works especially 1 and 2 are nil. PCOD 3 has also grated to a haft. Work is yet to resume despite the Cauvery holdup having long since abated. We will watch this October and November and recommend any further action other than termination, and in consonance to the contract.
