Entering data in to the Incidents and Log Entries pages in PowerSchool

When an incident occurs you will start by bringing up the student in PowerSchool and choosing the “Log Entries” link on the left side of the page. You will then select the “New” button on the “Log Entries” screen.

Area’s identified by a red arrow are required. Make sure to hit Submit!

To add Participants click on the green Plus symbol. After Clicking the plus sign the following box appears.

To add Behavior click on the green Plus symbol. After Clicking the plus sign the following box appears.

When documenting an Incident in PowerSchool you must: 1. Locate the student in PowerSchool by typing their last name in the search box on the start

page. 2. Select the “Log Entries” link from the left hand navigation window. 3. Click on the “New” button and fill out all required fields then hit submit. 4. Select the “Incidents” link from the left hand navigation window. 5. Click on the “Create New Incident” button and fill out all required fields the hit submit. 6. After all required steps have been taken we will be able to pull the Colorado Safety, Discipline

and Incident report from PowerSchool. 7. This data collection will run from May 2013 – June 2013. More specific dates will be available

in January 2013.
