Important Information The Centre opens at 7am and closes at 6pm each day listed above. Our closing time is 6pm SHARP and late collection

fees apply for pick-ups after this time.

There is no limit to the number of days you can book for Vacation Care – you can book for one day, every day or anything in between.

On excursion days, Departure and Return times are approximate. If you need an exact time, please contact Rachel on the day.

Please check your child’s exact school holiday dates, some school dates may differ.

I hereby confirm that I have enclosed a full enrolment form OR I have previously completed a full enrolment form and

now wish to re-enrol my child to attend the Vacation Care program

operated by Smileys Out of School Care on the days indicated on the attached form.

I am aware that the daily program as advertised can change without notice due to changes in weather conditions or

unforeseen circumstances at the venue, staffing or transport. I agree to any changes made within the programming of

the Vacation Care Program for July 2018. I am aware the full price is $89 per day should I not receive the Child Care

Subsidy (CCS). I am aware that once my enrolment form is received, those days are final and any days not attended

must still be paid for.

I give permission for Smileys Out of School care to take photographs of my child/ren with a digital camera and to use

these photos in displays within the centre and in the Early Works programming software. These images may be shared

with my permission in the daily journal and in observations where my child is in a group photo. These images will be

shared only with the families whose children are also in the same image. These images will be used solely for

documentation and programming purposes and will be kept confidential.

I am aware that there is a ratio of 1 educator to 10 children during this Vacation Care period, with a maximum of 40

children attending any given day. Activity plan and risk assessments are prepared for every day of the Vacation Care

program; available to parents on request.

I have read all of the information and agree to abide by the conditions as stated, including Bus Travel, as per the

attached program. Parents must ensure that all information regarding their child, emergency contacts, and medical

details are updated on this enrolment slip.
