TEMEKE MUNICIPAL COUNCIL
HEALTH DEPARTMENT INFORMATION BUREAU
1.1. Area and Location
Temeke District with an area of 240 sq. km and a population of 1,629,043 whereas 794,100 are males and 834,943 being females with a
growth rate of 4.6% annually. It is formed by 2 divisions i.e. Chang’ombe and Mbagala with 23 wards and 142 sub-wards (Mitaa)
respectively. It is on the Southern part of Dar es Salaam region with the Indian Ocean bounding it in the East, whereas Mkuranga District
(Cost region) in the South, Ilala District in the North and West.
1.2 Health Facilities in the District.
The District has 126 health facilities, the table below shows distribution of all facilities by type and ownership.
Health facility distribution in the district stretches as far as 28 km from the Municipal headquarters, the farthest being Mikwambe
Dispensary in Toangoma Ward. However the existing facilities are at least within 5 km radius to each other.
There are still 6 Ward which are under saved as far as Public service health facilities is concern. These Wards are Tandika, Kilungule,
Kijichi, Kurasini, Kibondemaji and Miburani. Kijichi Ward there is a Health Centre under construction and Kilungule has identified a piece
of land for Dispensary construction next financial year. .

To maintain and facilitate the maintenance of peace, order and good governance within its area of jurisdiction.
To promote the social welfare and economic well being of all persons within its area of jurisdiction.
Subject to the national Policy and plans for rural and Urban development to further the Social and economic development of its
area of jurisdiction;
To take necessary measures to protect and enhance the environment in order to promote sustainable development;
To give effect to the meaningful decentralization in political, financial and administrative matters relating to the functions,
powers, responsibilities and services of all levels of local government authorities.
To promote and ensure democratic participation in, and control of decision making by people concerned.

Main challenges;
During the implementation period Temeke Health Delivery System experienced some challenges as follows;
Maternal health condition was alarming due to fact that there was an increase of Maternal mortality ratio from 64/100,000 in
2015 to 90/100,000 (2016).
Bureaucracy in tendering and procurement procedures affects implementation of planned activities timely.
Inadequate budget to run Regional referral Hospital (Temeke) and Mbagala Rangitatu. The two hospitals are over congested
and the infrastructures are inadequate to accommodate them.
Frequent out of stock from the medical store department (MSD) also contributes to unavailability of medicines, medical
equipments and supplies in our facilities.
Lack of emergency unit and Intensive Care Unit (ICU) at Temeke Regional Referral Hospital, but now it is under construction
1.3.1. Climatic conditions.
Temeke district lies on the tropical coastal belt of Tanzania. It experiences high temperatures throughout the year ranging from 23 0c
degree (in May to August) to 35 0c (in September to April). The Municipality experiences heavy rains in the month of March – June.
During this season the community do experience outbreaks of waterborne and malaria disease.
1.3.2. Administration
Temeke Municipality has 2 divisions namely Chang’ombe and Mbagala which are further subdivided into 23 wards and 122 sub-wards
(mitaa). Health facility distributions in the Municipality have observed the administration boundaries to ensure that each
ward has at least one health facility.18 wards have a public health facilities while 6 do not have a public own facility but served by
private health facility. However some of the wards have more than one facility due to high population density. Densely populated
wards like Tandika and Kurasini are not served by any government facility, but being in the urban setting they are served by good
number of private facilities. Plans are underway to construct a dispensary at Tandika ward in the near future.
1.3.3. Transport and communication
Most part of the district is semi urban with rapid urbanisation in all wards. All areas are accessible by roads which are either tarmac or
all gravel roads. Popular communications services among others is the personal mobile phones and in some parts mostly urban areas
they are served by Internet services. Improved transportation and communication services has enhanced accessibility to all the
facilities and strengthened referral services.
1.3.4. Electric supply.
Currently, Temeke Municipality is experiencing rapid urbanisation with high demand of electric power. The ongoing power distribution
has covered all 23 wards. The improved power distribution coverage has enabled many health facilities to provide health services 24
hourly throughout the year.
1.3.5 Water supply:-
Water supply coverage in the Municipality has increased appreciably with more deep wells which are widely distributed, covering all
23 wards and their respective Mitaa. The Municipality is currently discouraging construction of shallow wells, and promoting proper
toilet empting with the use of Gulpers emptier whose tanks are small and easily carried in highly density populations areas. Nevertheless
Temeke residents are still getting water supply from City Water Mains though the supply is not constant. Water availability has greatly
influenced the reduction of waterborne related diseases like diarrhoea, cholera and dysenteries. Moreover the available wells are
treated by chlorine.
1.3.6 Social - Economic Activities:
Most people in the urban of the Municipality are mainly engaged in business, petty trading and industrial labourers. Due to its large size
many people from different parts of the city and country side have moved to the Municipality, hence new and heavy constructions of
infrastructure are taking place rapidly. Improved social and economic services have also enhanced the livelihood of the people of
Temeke hence enabling them to contribute towards improving health delivery system through cost sharing.
1.3.7 Education:-Schools distribution in Temeke Municipal Council
The Municipality has 126 primary schools of which 83 are public and 43 are private owned. There are 49 pre-primary schools are private
owned while the rest are public owned which are included in 83 primary schools). There are 61 secondary schools (26 are public and
35 are private owned). In view of the fact that health status and wellbeing of an individual depend largely on the level of
understanding or education of the respective society, this entails that the marked increase of education institutions is a fundamental
and core factor to health promotion and improved live hood of the people of Temeke. The more people are educated the more
responsible they will in taking care of their own health.
1.3.8 Population profile
Temeke Municipality has a population of 1,629,043 whereas 794,100 are males and 834,943 being females with a growth rate of at 4.6%
annually.

Population distribution has shown that there is rapid urbanisation amongst many wards in the Municipality that it has
necessitated the Government to subdivide the 32 wards to 2 Municipalities such as ;Temeke MC consist of 23 wards while 9
wards located to the new Kigamboni MC (2016). Azimio, Tandika, Miburani, Mbagala and Keko are most densely populated
wards. This population distribution pattern facilitates smooth and logical distribution of health facilities while observing
population density and boundary divisions.
Community and other stakeholders’ involvement
The new Municipal Council Health Board and Health Facility Governing Committees (HFGC) were inaugurated in November, 2014 and
will be in place for next 3 years. This is an important community organ that bridges the voice of the users to the health facility providers
in safeguarding the interest of the consumers and the service providers.
Responsibly the committees are the overall foreseer of all the services that are offered in their respective facilities. Among other
responsibilities are to ensure proper use of the facility resources both material and finance and participation in planning and evaluation
of implementation of the planned activities.
Partners in health service in the district.
There are several partners collaborating with the Municipality in rendering quality health care at various levels of health service
provision; a few to mention are included; PSI, UNICEF, PATHFINDER, MDM, CCBRT, MDH, and Muhimbili UCHS for others refer to table 8.
Review of the implementation of the last year’s plan:
Achievements.
Based on national package of essential health interventions and performance monitoring indicators for year 2015 (HMIS), among
others good performance was attained in the following areas;
19% of all cases attended in OPD were Malaria
Vaccination coverage for all antigens was attained at an average above 93%,
23 (100%) health facilities have functional health boards and health committees,
There is a dramatically reduction of TB case fatality rate from 9% to 7.5%
Construction of Kijichi Health centre is in final stage
Construction of Charambe dispensary at is in a final stage
Supervision coverage for both public and private health facilities was attained at 75%,
Health partner’s support in the Health service Delivery of system for Temeke Municipality includes;
1. Support from MDH comprised of Hospital supplies, furniture, reagents and renovation of health facilities.
2. Support from CCBRT on mentoring of health staff mainly in Obstetrics area.
3. Tanzania Breweries Ltd supported construction of deep well at Roundtable Health centre.
4. Malaria case management supported by Malaria Care and PSI
5. Pathfinder International supported training of health workers the use and insertion of Long term(mini lap)family planning
methods
Main challenges;
Some of the challenges that were experienced by the Temeke Health Delivery System during the implementation period are as follows;
While appreciating the approved number of human resource for health as per request for 2015/16 there is still high work load
Bureaucracy in procurement and tendering procedures are among the contributing factor to low performance of
implementation of the planned activities.
Frequent stock out of medicine, equipments and radiological supplies from MSD
Rangitatu and Temeke referral and regional hospital are still experiencing marked congestion of patients in most of the service
areas.
Way forward for the new plan 2016/2017
To continue recruitment of health staff of all cadres to cope with the existing and expected workload.
To abide to approved allocation of funds for medicine equipment and supplies to cover the MSD gap.

The purpose of the Comprehensive Council Health Plan
The new plan has engaged major interventions as addressed by the 13 health priority areas which are linked with the Temeke
Municipal Council main objectives as predetermined in its strategic plan for the year 2016/2022.
1. Continuing recruitment of new staff and improving retention of health personnel.
2. Strengthening measures to decongest Temeke and Rangitatu hospitals.
3. Quality of health services to reduce burden of diseases (mortality and morbidity) improved.
Interventions are planned to address the national health objectives which are linked with the council’s strategic objective plan and set
targets. However in this year’s plan the Ministry of Health and Social Welfare has been included in the BRN programme which is
planned to last from 2015 to 2018 with 4 major components with specific set targets.

Priority health activities are incorporated in the plan in an integrated approach and funding from all sources are well indicated to
enhance easy performance.
Planning Techniques and logistical support.
Through a bottom – up approach, identification of problems and priority setting were realised through Evaluations and Preplanning
meetings at all levels which were ultimately concluded in a common pre-planning meeting.
During the Planning phase community needs were realised through community representatives at ward level, Health board and health
facility governing committees, invited health partners i.e. MDH, PSI, MdM, Pathfinder, Sikika, NHIF, CCBRT etc.
