Adult SupportsIndividual Support for Adults and CarersWe provide individuals who are experiencing mental illness with the support needed to live a meaningful and valued life. We focus on what individuals can do, rather than what they can’t. Support is available in a range of settings, such as in the home or a chosen location in the community.

Flexible support options are available for carers and families of people with a mental illness, whose health and wellbeing is impacted by their caring role.

We provide supports to people living in Darwin - urban and rural regions, Palmerston, Katherine, Daly River and Maningrida.

Centre Based Support

The Day to Day Living Centre is a welcoming place, where participants can socialise and take part in a range of activities; such as cooking, art and community outings in a group setting.

Home Care Packages

TeamHEALTH’s Home Care Packages offer older adults tailored support so they can continue living independently in their own home.

Home Care Packages offer a range of supports, which can be tailored to meet individual needs.

To be eligible for a Home Care Package:

• Individuals need to be assessed and found eligible with My Aged Care or have an NDIS package that includes these supports

• Non-indigenous adults are required to be 65 and over and

Support is available in Palmerston-Litchfield region, Katherine and Gunbalanya. Referrals are accepted from any source.

Residential SupportsResidential Support is available for individuals who are experiencing severe mental health concerns. Support is provided to move towards independent living and includes meal preparation, attendance at appointments and social activities.

To be eligible for residential support, individuals are required to have a referral from Top End Mental Health Service (TEMHS).

We have two residential services:

The Manse: 24 hour, long term accommodation located in Parap, NT

Papaya: Short-term accommodation located in Nightcliff, NT

HousingWe provide non-crisis, medium to long-term accommodation options for 18 to 64 year olds and their family, who are living with a mental illness.

Individuals need to be engaged with a Mental Health Recovery Worker through TeamHEALTH or another approved provider and have a Top End Mental Health Services (TEMHS) Case Manager.

Education and TrainingTeamHEALTH are committed to delivering education and training to reduce the stigma and discrimination associated with mental illness and contribute to better mental health outcomes.

Our trained facilitators provide up-to-date, evidence based education and training for workplaces, schools and individuals in the community.

Training offered includes:

• Suite of Mental Health First Aid/Youth Mental Health First Aid

• SAFEtalk

• Understanding Trauma

Indigenous adults are required to be 50 years and over.

To discuss My Aged Care eligibility, please call them on 1800 200 422 or speak to a GP or Health Professional. To discuss receiving a Home Care Package or NDIS plan, please contact us.

Support is available in urban and rural Darwin regions.

Support Coordination for Individuals with an NDIS Package

Support Coordination is an NDIS support item, whereby individuals receive help to manage their NDIS package. A TeamHEALTH Support Coordinator will work with individuals, their family, friends, service providers and other health professionals to develop a detailed support plan. The support plan is designed so that individuals can achieve the goals in their NDIS package. We will help individuals understand the flexibility in their funding to make sure they get the most out of their plan.

Child and Family SupportsThe Child and Family Wellbeing Service works in partnership with families to support children and young people (ages 0-18), who are showing early signs of, or are at risk of developing mental health concerns. Support is practical and flexible and is available in the family home, at a TeamHEALTH location, or somewhere in the community that suits you.
