State of Ohio – Notice to Consumers Ohio Consumers Have the Right to Obtain a Security Freeze You may obtain a security freeze on your credit report* to protect your privacy and ensure that credit is not granted in your name without your knowledge. You have a right to place a "security freeze" on your credit report pursuant to Ohio law. The security freeze will prohibit a consumer credit reporting agency** from releasing any information in your credit report without your express authorization or approval. The security freeze is designed to prevent credit, loans, and services from being approved in your name without your consent. When you place a security freeze on your credit report, within five business days you will be provided a personal identification number or password to use if you choose to remove the security freeze on your credit report or to temporarily authorize the release of your credit report. To provide that authorization, you must contact the consumer credit reporting agency and provide all of the following:
(a) Information generally considered sufficient to identify the consumer; (b) The unique personal identification number or password provided by the consumer credit
reporting agency; A consumer credit reporting agency that receives a request from a consumer to temporarily lift a security freeze on a credit report shall comply with the request not later than fifteen minutes after receiving the request. A security freeze does not apply to circumstances in which you have an existing account relationship and a copy of your report is requested by your existing creditor or its agents or affiliates for certain types of account review, collection, fraud control, or similar activities. If you are actively seeking credit, you should understand that the procedures involved in lifting a security freeze may slow your own applications for credit. You should plan ahead and lift a freeze, either completely if you are shopping around, or specifically for a certain creditor, a few days before actually applying for new credit. A Summary of Your Rights Under the Fair Credit Reporting Act
The federal Fair Credit Reporting Act (FCRA) promotes the accuracy, fairness, and privacy ofinformation in the files of consumer reporting agencies. There are many types of consumer reportingagencies, including credit bureaus and specialty agencies (such as agencies that sell information aboutcheck writing histories, medical records, and rental history records). Here is a summary of your majorrights under the FCRA. C You must be told if information in your file has been used against you. Anyone who uses acredit report or another type of consumer report to deny your application for credit, insurance,or employment – or to take another adverse action against you – must tell you, and must giveyou the name, address, and phone number of the agency that provided the information.
C You have the right to know what is in your file. You may request and obtain all theinformation about you in the files of a consumer reporting agency (your “file disclosure”). Youwill be required to provide proper identification, which may include your Social Securitynumber. In many cases, the disclosure will be free. You are entitled to a free file disclosure if:
C a person has taken adverse action against you because of information in your creditreport;
C you are the victim of identify theft and place a fraud alert in your file; C your file contains inaccurate information as a result of fraud; C you are on public assistance; C you are unemployed but expect to apply for employment within 60 days.In addition, by September 2005 all consumers will be entitled to one free disclosure every 12months upon request from each nationwide credit bureau and from nationwide specialtyconsumer reporting agencies. C You have the right to ask for a credit score. Credit scores are numerical summaries of yourcredit-worthiness based on information from credit bureaus. You may request a credit scorefrom consumer reporting agencies that create scores or distribute scores used in residential realproperty loans, but you will have to pay for it. In some mortgage transactions, you will receivecredit score information for free from the mortgage lender.
C You have the right to dispute incomplete or inaccurate information. If you identifyinformation in your file that is incomplete or inaccurate, and report it to the consumer reportingagency, the agency must investigate unless your dispute is frivolous. See www.ftc.gov/credit for an explanation of dispute procedures.
C Consumer reporting agencies must correct or delete inaccurate, incomplete, orunverifiable information. Inaccurate, incomplete or unverifiable information must beremoved or corrected, usually within 30 days. However, a consumer reporting agency maycontinue to report information it has verified as accurate.

C Consumer reporting agencies may not report outdated negative information. In mostcases, a consumer reporting agency may not report negative information that is more than sevenyears old, or bankruptcies that are more than 10 years old.
C Access to your file is limited. A consumer reporting agency may provide information aboutyou only to people with a valid need -- usually to consider an application with a creditor,insurer, employer, landlord, or other business. The FCRA specifies those with a valid need foraccess.
C You must give your consent for reports to be provided to employers. A consumerreporting agency may not give out information about you to your employer, or a potentialemployer, without your written consent given to the employer. C You may limit “prescreened” offers of credit and insurance you get based on informationin your credit report. Unsolicited “prescreened” offers for credit and insurance must includea toll-free phone number you can call if you choose to remove your name and address from thelists these offers are based on. You may opt-out with the nationwide credit bureaus at 1-888-5-OPTOUT (1-888-567-8688).
C You may seek damages from violators. If a consumer reporting agency, or, in some cases, auser of consumer reports or a furnisher of information to a consumer reporting agency violatesthe FCRA, you may be able to sue in state or federal court.
