    University (collectively referred to as UNIVERSITY):In consideration for _____________________ being allowed to participate in said activity, it is agreed that all risks attendant to watching and/or participating in camp

    activities, including, but not limited to bodily injury, are assumed by the camper and his/her parents and/or legal guardian as indicated by the signature hereto. I hereby certify that theabove named camper is physically able to participate in the camp and that I know of no physical impairments which would in any manner limit his/her participation in the program.

    I, for myself, and on behalf of my child, hereby release, waive and discharge UNIVERSITY, its instructors, agents and employees from every claim, liability or demand of any kind sustained,whether caused by the negligence of the UNIVERSITY or otherwise. This release shall be binding upon any heirs, administrators, executors and assigns of mine. I further agree to indemnifythe UNIVERSITY from any loss, liability, damage or cost it may incur due to my participation in said activity in any way whether caused by the UNIVERSITY or otherwise. I also certify thatI am the legal parent or guardian of the above named camper and have full right to provide this release.

    In the event of illness or injury resulting or arising directly or indirectly out of said activity, I hereby give my consent and authorization for (1) the administration of emergency first aidcare and treatment at the scene of an emergency by faculty, staff members or volunteers of UNIVERSITY or (2) the administration of any treatment deemed necessary by a licensed physicianor dentist and (3) the transfer to any hospital reasonably accessible. This authorization is not intended to cover major surgery unless the medical opinions of two (2) licensed physicians ordentists, concurring in the necessity for such surgery, are obtained prior to the performance of such surgery.

    ONE-DAYALL-POSITION CAMPMonday-Friday June 6-10 $304 to 6 p.m. Grades 9-12This ast-paced two-hour camp will give the YSU coaching sta achance to assess your talent and give you a eel or what it is like tobe part o a practice. Campers may attend more than one workout.

    This is a great chance to show the coaching sta what you can do.

    QB/WR/TE SKILLS CAMPMonday, June 13 $508:00 a.m. to 4:00 p.m.Grades 9-12This camp is intended to oer a unique learning experienceor quarterbacks, wide receivers and tight ends where teach-ing is emphasized and individual improvement is the goal. Alldrills and flm sessions will be conducted by the YSU coachingsta.Additional Information: Lunch will be provided.

    Quarterbacks should bring a ootball with their namewritten on the ball.

    Monday, June 20 $459:00 a.m. to 3:00 p.m.Grades 9-12Former YSU and NFL placekickers Je Wilkins and Paul McFaddenwill work with punters, placekickers, and long snappers to improveall acets o their game. Learn rom two o the best in our programshistory in an inormative setting.

    COACH WOLFORD & STAFF

    YSU Head Football Coach Eric Wolord, a graduate o Ursuline High School, built an impressiveresume serving as a collegiate assistant or 16 years prior to taking over at YSU in 2010. O those, 12were spent on the FBS level.

    As an assistant, he has coached or Steve Spurrier, Ron Zook, Mike Stoops, Darrell Dickey,Dana Dimel, Jim Leavitt and Bill Snyder.

    In his frst year as the Penguins head coach, Wolord, a non-stop worker, has turned the mind

    set o the program into playing a hard-nosed, exciting brand o ootball that YSU ans can be proud o.In 2010, the Penguins had a record-setting campaign on oense. YSU averaged a school-best 412 yardsper contest while averaging 31.3 points per game, the fth highest total in school history. During theyear, the Guins gained at least 500 yards three times - a frst in the programs history.
