Overview Aloysius Butler amp Clark is working with the Health Care
Commission and the DCHI Clinical Committee to develop a provider
engagement strategy specific to the Practice Transformation initiative
Objectives

Update
Dr Lee attended the June Clinical Committee meeting to enhance awareness of
HIT offerings available to the provider community through DHIN Discussion also
revolved around the Common Scorecard provider engagement and long-term
sustainability plans to ensure the continuation of key HIT initiatives

Both the DHIN and Clinical Committee found the conversation to be helpful and
will look towards continued collaboration moving forward to foster discussion

revolved around the Common Scorecard provider engagement and long-term
sustainability plans to ensure the continuation of key HIT initiatives

Provider Engagement Strategy
Overview Aloysius Butler amp Clark is working with the Health Care
Commission and the DCHI Clinical Committee to develop a provider
engagement strategy specific to the Practice Transformation initiative
Objectives
bull Increase awareness of Practice Transformation challenges and
benefits on a local and national level
bull Increase information sharing between practices enrolled
bull Identify successes and challenges of reaching the milestones
bull Create educational opportunities that can be used to ease
transformation milestones and help with onboarding new enrollees
Target Audience
bull Primary Target Audience Primary Care Providers and staff members
of practices enrolled in Practice Transformation including ACOrsquos
bull Secondary Target Audience Primary Care Providers and staff
members of practices that are not enrolled in Practice Transformation
