中文摘要

随着我国人口老龄化进程的加速，老年人口的养老问题已经引起了全社会的

共同关注。然而由于家庭养老功能的弱化，机构养老发展的不足，为了有效应对

老年人的养老需求，居家养老模式应运而生。目前学者们更多的关注推行居家养

老模式的必要性，居家养老模式的探索、运行机制、面临的困境及对策，和社会

工作如何介入居家养老。但是这些研究多数停留在理论层面，提出的对策及介入

策略也过于宏观，缺乏实际的可操作性。因此，本文以 X 市 Q 社区为研究对象，

采用个案研究的方法，以社工站的居家养老实践为基础提出了外展社会工作在居

家养老服务中应用的可能性。

根据 Q 社区社工站的居家养老服务实践，笔者发现外展社会工作可以与居

家养老服务相结合，并且分析了外展社会工作能够在居家养老服务中的应用的原

因。另外，笔者提出了外展社会工作在居家养老服务中应用的具体策略，并将其

划分为五个阶段：（一）前期准备阶段：在开展工作前先对外展社会工作者进行

分组及对拟工作区域进行划分，每个小组负责一个固定的工作区域；（二）发现

服务对象阶段：作为外展社会工作者一定要明确服务对象的特点，根据服务对象

的生活习惯发现服务对象；（三）接触服务对象阶段：外展社会工作者要用自己

的真诚和行动取得服务对象的信任。与老年人初次接触的时候要让对方感受到社

工对他的关怀，而不是带有目的性的接触；（四）沟通、交流阶段：外展社工在

与服务对象沟通、交流的过程中要向他们传达一种信息——我们愿意帮助他们，

同时愿意与他们共同探讨解决问题的方式；（五）介入阶段：通过前几个阶段的

铺垫，外展社工就可以用最为合适的方式回应服务对象的需求了。

Abstract

With the accelerated process of population aging in China, the pension problems

of the elderly have caused the common attention of the whole society. However, due

to the weakening of family pension function and lack of institutional pension

development, in order to effectively deal with the pension needs of the elderly,

home-based care service comes into being. At present, scholars pay more attention in

the necessity of the home-based care for the elderly, mode of exploration, operation

mechanism, difficulties and countermeasures of the home-based care, and social work

how intervene in it. But these studies most stay in the theoretical level, the

countermeasures and intervention strategies in these studies is also too macro and lack

of practical operability. Therefore，the article takes the Q Community of M City as the

research subject and using case study method, based on the practice in social work

station, and then puts forward the possibility of outreaching social work that can apply

in home-based care services.

According to the practices of the home-based care services carried out by social

work station, the author discovered that outreaching social work can be combined

with home-based care services, and analyzed the reasons why outreaching social work

can be applied in it. In addition, the author proposes specific strategies for how

outreaching social work applies in the home-based care services, and it can be divided

into five stages: (a) Preparation stage: outreaching social workers to work in groups

and proposed work area to be divided, each group responsible for a fixed work area;

(b) Found service object stage: as outreaching social workers must make clear the

characteristics of the object, and according to the habits of the object found them; (c)

Contact with the service object stage: outreaching social workers need to use their

sincere and action to achieve the trust of the object. And the initial contact with the

elderly needs to make them feel the care of social workers, rather than with the

purpose of contact; (d) Communication and exchange stage: in the process of

communicate with the service object, outreaching social workers would like to send a

message to them——we are willing to help them, and together with them to explore

the problem-solving approach; (e) Intervention stage: through the matting of the first

few stages, outreach social work can use the most appropriate way to respond to the

needs of the service object.
