Selen
•1817 im Bleikammerschlamm einer
Schwefelsäurefabrik entdeckt.
•Als essentielles Spurenelement in der
biogenen Aminosäure Selenocystein
enthalten.
•Spielt wegen seiner hohen Reaktivität mit
Sauerstoff eine wichtige Rolle beim
Schutz der Zellmembranen vor oxidativer
Zerstörung (Radikalfänger).
•Am Katalysemechanismus weiterer
Enzyme beteiligt und in vielen Proteinen
enthalten, deren Bedeutung noch nicht
geklärt ist.

Selenoproteine
• Glutathionperoxidase 1 - V
• Thioredoxinreduktase (TrxR)
• Iodthyronin-5'-deiodinasen
(Schilddrüsenhormondeiodinasen) (DIO-I, DIO-II, DIO-III)
• Selenoprotein P (Se-P) (Transportprotein)
• Selenphosphatsynthetase → Synthese von
Monoselenophosphat, (Vorläufer von Selenocystein)
• u.a.

Selenmangel
• Erkrankungen aufgrund Mangelversorgung mit Selen nur in Ländern mit
extremer Selenunterversorgung (Nordkorea, Nordostchina).
• Keshan - Krankheit (juvenile Kardiomyopathie)
• Selenmangel >> Mutation des harmlosen Coxsackievirus B3, das
dadurch virulent wird
• Kaschin-Beck-Krankheit des Menschen (nutritive
Gelenkknorpeldegeneration)
• Epidemische Neuropathie des Menschen
• Selenmangel verursacht eine Mutation des Influenza-
A/Bangkok/1/79-Virus

Trials using more than 500
μg per day of selenium
showed a trend towards a
lower mortality (RR = 0.80,
95% CI 0.63 to 1.02, P =
0.07) whereas trials using
doses lower than 500 μg
had no effect on mortality
(RR 0.94, 95% CI 0.67 to
1.33, P = 0.75).

Selenium
supplementation
for critically ill
adults (Review)
Allingstrup M et al. Cochrane Library 2015, Issue 7
Despite publication of a number of
trials, the current evidence to
recommend supplementation of
critically ill patients with selenium
or ebselen remains disputed.
Trials are required which
overcome the methodological
inadequacies of the reviewed
studies, particularly in relation to
sample size, design and
outcomes.

sepsis
Sakr et al. Critical Care 2014, 18:R68Retrospective analysis of all adult patients admitted
with severe sepsis 1/2004 and 4/2010
In this retrospective analysis of a large cohort of surgical ICU
patients with severe sepsis, adjuvant Se supplementation in the
form of sodium selenite had no impact on in-hospital death rates
after adjustment for confounders.

SodiUm SeleniTe Adminstration IN Cardiac Surgery
(Sustain CSX®-Trial)
•As outlined before selenium is considered the
cornerstone of antioxidant defense mechanism and may
be one of the most important antioxidants, which may
attenuate the perioperative inflammation and reduce
development of organ dysfunctions that frequently
determines a prolonged ICU stay 10. Yet, only limited
data obtained from small cohorts of patients exists that
highlights the crucial role of this trace element in cardiac
surgery patients.
•Our primary outcome for this trial is “persistent organ
dysfunction+death, which will be evaluated as a
composite endpoint.PD Dr. med. Christian Stoppe - Personal communication

Schlussfolgerungen
• Selen: essentielles Spurenelement - von Relevanz für
biologische Prozesse im Rahmen der Immunabwehr !
• Bei kritisch Kranken: inverse Assoziation zwischen Selen -
Plasmakonzentration und Mortalität (v.a. bei Sepsis) !
• Kleinere klinische zeigen teilweise positive Effekte einer
Hochdosis - Selen Therapie !
• Dieser Effekt bestätigt sich nicht Meta-Analysen !
• Perspektivisch gilt es besser herauszuarbeiten, welche
Patienten profitieren, und welche nicht !
