3.1 Anti-hormonale behandeling
• Hormonen : geproduceerd door organen, uitgescheiden in bloed, regelen biologische processen• Vb Geslachtshormonen : oestrogeen, progesteron, testosteron
• Hormonen en kanker : Kankercellen die oorspronkelijk ontstaan zijn uit een hormoongevoelig orgaan kunnen receptoren dragen voor dat hormoon ‘hormoongevoelig’ (beschrijving in APO verslag)
• Bij contact met dit hormoon krijgen de cellen een groeistimulus dat via signaaltransductie naar de kern wordt doorgegeven
• dit systeem afremmen door• Hormoonproductie stil te leggen• Blokkeren van hun werking thv de cel
• Perorale therapieën worden meer en meer intensief begeleid door verpleegkundigen :• Informatie/educatie. Zoveel mogelijk gebruik maken van brochures,
illustraties, …• Instructies voor inname. Aandacht voor therapie-trouw!
• Bevragen van nevenwerkingen, adviezen over eventuele symptoomverlichtende maatregelen
3.1 Anti-Hormonale therapie : verpleegkundige aandachtspunten
VOORBEELDEN uit de URO-ONCOLOGIE:- Decapeptyl, Firmagon, Depo-Eligard- Enzalutamide, Apalutamide, …- Zytiga
- …… BOOMING BUSINESS
• Komt in de bloedbaan terecht
• Verspreidt zich via de bloedbaan door het hele lichaam (nevenwerkingen!)
• Kan op vrijwel alle plaatsen in ons lichaam de kankercellen bereiken (metastasen)
• Remt de celdeling ahv verschillende mechanismen, waardoor deze cellen uiteindelijk vernietigdworden. Telkens door interferentie met het delingsproces (invloed op DNA en/of anderestructuren in de cel)
• Gevoeligheid van kankercellen voor chemotherapeutica kan verschillen
• Daarom combinatieschema’s
• Daarom aanpassing van een schema na verloop van tijd

Er bestaat geen relatie tussen de ernst van de bijwerkingen en het resultaat van de chemotherapie

3.3 Targeted Therapy – Doelgerichte Therapie
Door hoog selectieve farmacologische manipulatie worden intracellulaire signaaltransductiewegen die een belangrijke rol spelen in de celproliferatie, migratie, differentiatie, maturatie, angiogenese en geprogrammeerde celdood, therapeutisch beïnvloed. De componenten van deze signaaltransductiecascade zijn meestal veel meer aanwezig in de kankercel dan in de gezonde cel.

• Meest voorkomende histologie = transitioneel cel carcinoma (in westerse wereld)
• 25% van patienten heeft spierinvasief carcinoma dat later aanleiding zal geven (of reeds bij diagnose) tot metastasen
• Oorspronkelijke respons op chemotherapie over het algemeen goed
• Mediane overleving met combinatie chemotherapie bedraagt slechts 15maanden, (tov 6 maanden vooraf aanontwikkeling moderne chemo regimes), 5-jaars overleving bedraagt 15%

Voor het immunotherapie tijdperkCasus 1 : man, 60 jaar
• 11/2010: TUR blaas. APO: invasief transitioneel celcarcinoma T2G3.
• 12/2010: radicale cystoprostatectomie met vervangblaas.
• APO wijst op T3aG2/3.
• 2/2013 spoedgevallendienst omwille van abdominale pijn en hematurie
• Ct thorax en abdomen toont een massa prevesicaal links verdacht voor lokaal recidief; bovendien twee verdachte leverletsels en multipele kleine longletsels verdacht voor longmetastasering.
• 22/2/2013 : punctie van de lever
• APO :weinig gedifferentieerd carcinoma compatibel met een transitioneel celcarcinoma.
• 12/3/2013 : Carboplatin en gemzar
• 6/2013 en 9/2013 : duidelijke afname van de metastasering. Chemo stop 11/2013.
• 9/2014 progressieve ziekte : start Cisplatinum en Doxorubicine 4 cycli.
• Stop 11 11/2/2015 wegens subjectieve en hematologische toxiciteit
• 4/2015: progressie van de levermetastasering.
• 24/4/2015 start Taxol wekelijks
• 7/2015 : toename van de levermetastasering.
• 10/7/2015 : start Gemcitabine
• 10/2015: partiele remissie, gemcitabine verder
• 4/2016: progressie thv retroperitoneale klieren en thv lever.
• 02-06-2016 tem 20-06-2016: Palliatieve RadioTh thv retroperitoneale klieren

• 01/2015: botscan toont multipele botmetastasen, CT-abdomen levermetastasen en retroperitoneale adenopathieën.
• Op PET vermoeden van recidief TCC thv de rechter ureter
Voor het immunotherapie tijdperkCasus 2 : man, 55 jaar
• APO leverbiopt: infiltratie door urotheelcelcarcinoma
• 18/3/2015: start Xgeva en Carboplatinum-Gemcitabine, 9 cycli in totaal
• 13-11-2015 : progressie thv lever en bot
• 20-11-2015 : start Cisplatinum mono 50 mg/m² q2weken
• 15-2-2016: bilan na 3 maanden Cisplatinum toont progressie thv levermetastasen en van een adenopathie in de rechterfossa. Stop Cisplatinum, Start Taxol weekly
• 6-5-2016: partiële remissie na 3 cycli Taxol weekly. Verder Taxol.
• 03-08-2016: controle CT thorax-abdomen. Progressieve ziekte ter hoogte van lever en bot.
• 31-8-2016 : gesprek met familie, patient te zwak om naar
het ziekenhuis te komen. Enkele dagen later sterft hij
In het immunotherapietijdperkCasus : man 60 jaar• 19-01-2012: TUR-blaas uitgebreid pTa G1-2
• 04-06-2012: TUR blaas: matige dysplasie en pTis. Blaasspoelingen met BCG.
• 08-2016: macroscopische hematurie.
Cystoscopie toont een zeer
volumineuze en multifocale blaastumor
• 04-08-2016: TUR Blaas: papillair urotheelcelcarcinoma, laaggradig. pT1.
• 23-08-2016: cystoprostatectomie met Brickerderivatie.
APO: invasief papillair urotheelcelcarcinoom van de blaas, pT1N0, laaggradig
• 04-05-2017: opname naar aanleiding van acuut nierfalen obv bilaterale hydronefrose, waarvoor plaatsen van bilaterale nefrostomies
• 24-05-2017: 3 nieuwe longletsels en enkele licht vergrote klieren in het retroperitoneum en klein bekken
• 08-06-2017: CT-geleide longbiopsie. APO: compatibel met metastasen van het gekend blaascarcinoma
• 12-2017: mooie regressie na zes cycli carboplatinum - Gemzar
• 03-2018: progressieve ziekte.
• 23-3-2018 : start pembrolizumab
• 6-2018 : complete remissie
• 8-2018 : oplopende lipasen, hoogabdominale pijn en steatorree : sterk vermoeden auto-immune pancreatitis

In het immunotherapietijdperkcasus 2 : man, 78 jaar• 25-04-2017: hooggradig papillair urotheelcelcarcinoma. pT2
• Staging toont adenopathieën zowel lokaal als op afstand tot retroperitoneaal en vermoeden longmetastasen
• 18-7-2017: start Carboplatinum-Gemzar
• 7-11-2017 : stop chemotherapie, partiele remissie
• 28-3-2018: progressie ter hoogte van retroperitoneale en illiacale klieren Tevens progressie ter hoogte van blaastumor met bloeding. Start pembrolizumab.
• 6-2018 : massieve tumorprogressie, patient sterft enkele weken nadien.
