Team Leader

Name: Co, Lorenzo Angelo G. CoResponsibilities:

1. Do proper delegation of nursing responsibilities to each team members

2. Check the patients chart from time to time (every 30 minutes to 1 hour) for any new orders from the doctor.3. Conduct team conferences with the team, prepare for the duty ahead4. Listen to the endorsement report, take notes important details endorsed.5. Monitor team members (bedside nurses and medication nurses) from time to time6. Together with the medication nurse, check medication sheet and make sure that all medications of the patient are properly reflected and will be given to the patient on time

7. Follow up with the team, if the IV has been checked correctly and regularly (every 30 minutes)8. Assist the medication nurse in IV monitoring hourly

9. Assist the medication nurse in giving the medication and make sure that the 10 Rs are done10. Assist bedside nurses in nursing procedures done to the patient11. Remind the bedside nurse to do preliminary charting at/before 10am

12. Make sure that all team members (or at least 1 team member) is/are available during the doctors rounds13. Make sure that the patients vital signs are taken at the correct time (8:00 am, 12:00 noon) and charted properly in the jot sheet, patients chart, nurse-notes)14. Listens to the concerns of the team members and always be ready to assist them in any way.

15. Assist the bedside nurses in monitoring the patients16. Inform the clinical instructor if there are procedures to be done17. Follow up bedside nurse to do final charting before 2:00pm

5. Do morning care including bed bath, bed making and nail/foot care if needed

6. Do preliminary charting by 9am, let the clinical instructor check it and after write it on the patients chart

7. Plot the vital signs in the patients chart immediately after getting, to avoid delay

8. Monitor and measure the input and output of the patient specially9. Know what meds are given: Stat, prn, starting meds, IV therapy to follow-up

10. Prepare the play therapy that is age appropriate.

11. Provide emotional and psychological support to the patient/ patients family12. Interact with the patient to build rapport and establish trust through play therapy13. Make sure that all bedside care procedures and final charting has been done before 2:00pm

14. Provide comfort and adequate rest to the patient

15. Check from time to time patients chart for new order and new information
