Infantry soldiers may soon wear a harness with an external spine that bears the heavy loads of ammunition, batteries, water and

other combat necessities they carry into battle. The Integrated Support Exospine is a rigid yoke-belt system that is designed to redistribute the weight of heavy combat equipment so it’s much more comfortable to carry over long periods of time. Infantry officials from Fort Benning, Georgia, say that this carbon fiber, load-bearing device showed “great promise” when soldiers tested it last fall during the Army Expeditionary Warfighting Experiment there. “It takes the load off the soldier’s shoulders and lower back and redistributes it to hips and big leg muscles,” said Col. Daniel Barnett, director of the Soldier Requirements Division at Benning’s Maneuver Center of Excellence. “The soldier feedback on that is ‘this is huge.’ ” The Exospine is not ready for combat use, but infantry officials say it has the potential to reduce the injuries and fatigue that come with humping heavy gear on extended combat patrols. Evaluating ways to help soldiers manage weight is part of a larger effort to address shortcomings in the current squad formation identified in a recent Capabilities-Based Assessment conducted at Benning last year. The CBA was completed last year and found 22 “capability gaps” that hinder the nine-man infantry squad’s ability to be decisive in small-unit actions, infantry officials told Military.com. “This is really a new approach with the squad,” Barnett said, describing how similar efforts in the past have focused on improving the effectiveness the Army’s brigade combat teams. The effort will focus on areas such as force protection, lethality and situational awareness. The Army has made progress in reducing the individual load soldiers carry with lighter body armor, weapons, and other kit. But some battlefield essentials such as bullets, mortar shells and radio batteries are a long way from becoming significantly lighter. So infantry officials are searching for ways to make these burdens easier to carry, allowing soldiers to retain more energy for potential fights. The Exospine technology, invented by Emerald Touch Inc., features a highly flexible spine, designed to let soldiers move naturally while carrying a combat load when walking, running or engaging with equipment. But it’s not quite ready yet, Barnett said. The design needs to be altered so it doesn’t hinder critical combat tasks. “It’s hard for the soldier to fire from the prone because he can’t get his neck back far enough,” he said. Other soldier-load technologies tested in the AEWE, however, were less impressive. Infantry officials tested two versions of a six-wheeled cargo-carrying unmanned ground vehicle designed to carry rucksacks, water and ammo. Soldiers found several problems with both the Lockheed Martin Squad Mission Support System and the Carry-all Mechanized Equipment Landrover, made by Northrop Grumman. Both systems have a long way to go,” Barnett said. “None of the soldiers said they would take them to combat. They’re too slow. They’re too loud. And they have a high thermal signature.” Both vehicles are designed to navigate across rough terrain while shadowing formations of combat troops. “Fully loaded, once off an improved path, they tend to turn over and slow the speed of the squad,” Barnett said. New equipment is not the only answer. Infantry officials

are also looking at ways to ensure squads are less likely to walk into an enemy ambush. “We want to reduce their chances of being surprised,” Barnett said. To do that, the squad needs to have more access to combat “enablers,” such as unmanned aerial vehicles that gather information about enemy positions that can only be spotted from above, said Pat Berger, deputy director of Capability Management at Benning.

L4-5 Disc Herniation with Motor Weakness: Relief with Cox Technic

Introduction / History / Examination / Imaging: This 42 year old white female is seen for low back and left leg pain with Grade

3 of 5 left dorsi-flexion of the foot at the ankle and Grade 1 of 5 extension of the extensor halllucis longus tendon of the great toe. She has been seen by two orthopedic surgeons who have scheduled her for a selective foraminal steroid injection. An MRI, shown in Figures 1 and 2, was ordered on her first visit due to the motor weakness. The MRI shows on sagittal image loss of signal intensity at the L4-L5 disc with a large left paracentral disc prolapse that contacts the cauda equina and the left L5 nerve root.

• Treatment Plan and Goal: Long y axis decompression with flexion distraction of the L4-L5 disc space is instituted with Protocol 1. This is followed by positive galvanism for 12 minutes and tetanizing bilateral electrical stimulation at the L4-L5 level, the left posterior retrotrochanteric space, and the anterior tibialis muscle. Home care consists of ice to the low back for 30 minutes two times a day.

• Outcome: On day two after the first treatment, the dorsi-flexion strength at the left ankle for 4/5 and of the great toe 2/5.

• Conclusion: This case was very closely monitored for any progressive neurological deficit that would necessitate surgical discectomy. After treatment from October 20, 2011, through December 23, 2011 (12 visits), there was gradual relief of the left lower extremity pain with return of motor power of the left great toe dorsiflexion and dorsiflexion of the foot until her pain was VAS zero and motor power 5/5 at the ankle and 4/5

at the great toe. As of December 23, 2011, this patient has not been seen because the number of visits allowed by her insurance company expired, and she would not fund her own care. Luckily, her insurance coverage equaled her required treatment.

Myofascial Trigger Point Dry Needling is the

most powerful clinical tool for the treatment of soft tissue subluxation. The soft tissue subluxation (STS) is to be differentiated from the synovial joint-based subluxation treated by the chiropractic adjustment. Just as loss of motion and fixation is integral to the concept of synovial joint subluxation, so too is dry needling integral to treatment of the soft tissue system, albeit a different path. Just as we use the adjustment

to clear the fixation and impedance in the osseous system so is the dry needling technique equivalent in its power to clear the soft tissue fixation, its pathomechanics and biochemical imbalance. In the soft tissue scenario you have macro trauma, micro trauma, acute and chronic catalysts just as in the synovial/osseous system that lead to a sequelae of biochemical/biomechanical events. These events can foment spasming, locking and shortening of soft tissue elements that can no longer respond to internal cues and many external attempts to restore function to a normal physiologic state. The theories proposed by Simons and Travel serve as the underpinning of understanding that what has been coined a trigger point is only the extreme pathologic manifestation of the soft tissue subluxation which occupies a broader area under the continuum of soft tissue dysfunction. We can now explain clearly that the soft tissue becomes subluxated, locked and fixated just as the synovial system does. Reducing nervous system dysfunction via the soft tissue system is as critical as relieving it via the joint system. The work of Simons and Travel has contributed greatly to the understanding of this mechanism of muscle and soft tissue fixation. And other authors such and C. C. validate the chiropractic model but lend further support for nervous system dysfunction in the peripheral system throughout the soft tissues. And yet other authors such as Helene Langvine have written extensively about the broad communication of the fascial system and its effects on the body. Dry needling is a critical tool in treating these problems.

Direct and Effective The approach of using fine solid needles to unlock and release the soft tissue structures is more direct and effective than other techniques available today. Those that I have trained and I have seen very positive clinical outcomes. Patients who walk in antalgic walk out greatly improved and chronic patients that previously were unresponsive now respond completely or stabilize. This is occurring at the front lines of practice and we have a number of case studies from the referred literature that I have posted on my website. Sadly, only the doctors in sixteen states: Maryland, Delaware, Virginia, North Carolina, South Carolina, Alabama, Connecticut, Florida, Utah, Illinois, Colorado, New Mexico, Rhode Island, New Hampshire, Oregon and Texas are currently permitted to take advantage of this fantastic approach.

Doing the Right Thing the Wrong Way Can Cost You!

Yes, I know. Sounds strange doesn’t it? Strange as it sounds, it is far too familiar to those of

us who work with doctors on a daily basis to help improve their documentation, billing coding and collections. We see examples of doing the right thing the wrong way every day in chiropractic clinics across the nation. It happens when doctors attempt to do the right thing in helping patients with no insurance or limited benefits by “tweaking” their documentation and coding to result in an overall lesser charge for the patient. Be honest. Have you ever had a patient that required more than a routine workup but you used a lower level exam code because you knew they had no insurance coverage? Perhaps a 99202 instead of a 99203? No big deal right? WRONG! Or, as long as we are being honest, have you had a patient that you adjusted full spine, 4-5 levels, and you only charged a 98940 for 1-2 levels? No big deal right? WRONG! Ok, let’s try one more. How about this scenario? Have you ever had a Medicare patient that required a detailed workup and x-rays and you charged a lower level exam code or just billed for 2 x-rays when you actually performed 4, because you know Medicare doesn’t cover your exams or x-rays? No big deal right? WRONG! All of these are examples of down coding your services in an attempt to strike a balance between what insurance coverage may allow and what you feel your cash or underinsured patients can afford. We all know if you don’t perform a service you should not bill for it…it is called fraud. However, few may be aware that if you DON’T report what you did and DON’T charge for services you

would normally bill to insurance because you are seeing a cash patient, there is also the potential for fraud because of a dual standard of care and a potential dual fee schedule. Not to mention, down coding or failure to bill Medicare patients can be considered an inducement and subject to serious fines and penalties according to the Office of Inspector General and CMS. The bottom line is that down coding can be perceived as illegal and inappropriate just as up coding is when it is used to allow you to bill differently based on whether the patient is insured or a cash patient. It is “gaming” the system pure and simple. It is doing the right thing to help the patient, but it is being done the wrong way and it can cost you. What drives doctors to down coding or not charging for all their services? It’s really quite simple. There is the need to maximize reimbursements by billing your UCR fees or contract rates to insurance companies, but you haven’t found a good way to make care as affordable as you would like for your cash and underinsured patients. So the only tool you have is to “tweak” the documentation, coding or billing to allow you to do what you would like to do. It really is that simple, but it can REALLY be very costly if you are audited! So how can you document as you should, bill as you should and code properly for maximum reimbursement when there is insurance available and still be able to help the cash patients? UCR clinic fees. It allows you do what you are attempting to do now…help the cash or underinsured. But, with a network contract, you can do it without “tweaking” your documentation, coding and billing and you avoid running afoul of dual fee schedules or illegal inducements. So, do the right thing! Document properly. Code properly. Bill properly. And, consider joining one of the cash discount plans that allow you to “contract” with cash and underinsured patients. The contract model is not new. The contract model is what allows you to contract with multiple insurance companies for different rates for the same codes without it being considered a “dual fee schedule”. Cash discount plans simply provide you the protection of a “contract” and allow you to document, code and bill properly and still pass on some savings to your cash and underinsured patients. If you are documenting properly, coding properly and billing properly, good for you! Just make sure you don’t do the right thing the wrong way by trying to use a “bookkeeping reduction” or Time of Service discount that can be open to interpretation as to what is a “reasonable” discount. Far too many clinics use these tactics or strategies improperly in trying to help cash and underinsured patients. Most all patients are familiar with “buying clubs” like Sam’s Club and networks. Experience shows they don’t hesitate to join these to save money! A good cash discount plan solves so many of the potential problems for you and really helps the patient…the RIGHT WAY!
