    I. ACKNOWLEDGMENT

    The Completion of this OJT Practicum would not be finished without the

    their helping hands during the whole practicum.

    I would like to extend my warmest gratitude to the people that help me,

    strength to finished this On-the-job-Training, without you I wouldnt be able to

    finished this all.

    II. INTRODUCTION

    Me and my classmate Judy Ann didn't planned to have our practicum in

    Philippine Heart Center because our first choice was in the small private hospi-tal near Commonwealth Avenue, where Judy Anns godfather is working but

    suddenly they didnt accept trainees in that hospital. We our influenced to go

    in PHC by Diole Ann and Claudine because they said that PHC had a good en-

    vironment and it was true but we doubt to have our on the job training there

    when we knew that the practicum there has a payment amounting 16 php per

    day so we have to pay 600 php to complete 300 hours but since it will be pay

    in the last day of the training we just accept it because we dont have time to

    look for public hospital because most of them are loaded in trainees

    IV. OBSERVATIONS IN THE COMPANY

    What I have observed at Philippine Heart Center when I first saw it or

    start my training is there facilities are very clean, they have a well organized

    staffs and environment.

    The strength and challenges of this company is that they have a goodand best doctors, equipment and staffs. They are able to manage the differ-

    ent types and moods of patients and they have a good procedures in terms

    of paying, admitting services etc.

    I think the best practice of this company is their cleanliness and orderli-

    ness in their place and they have a well organized procedures in the com-

    pany.

    My over-all observation in Philippine Heart Center is they are very cleanand order and they possesses a good manner and patience to their co-

    workers and to the patients and its relatives

    What we do daily are the following:

    Accompany patients to Assistant Director Administrative

    Services to approved their papers for admission

    Encode in MEDTRAK, Photocopy Papers

    Logout papers to the other departments

    What we do weekly are the following:

    The things we do daily are the same as we do weekly .

    What we do monthly are the following:

    Record the DTR of the doctors to be passed on HRD, File the

    Surgical Operations in Cardiovascular Surgery, Encode the mortality Report

    for the month, Record the number of patients that when through surgical

    Operations for Adult Cardio and Pedia that will be passed on DOH.

    During my whole training in Philippine Heart Center , I think the prob-

    lem/s that I have encountered is in my personality because Im a shy and

    quiet person so I didn't talked to much in the office and Im having a hard

    time to focus on my task.

    VII. SELF-ASSESSMENT

    A. Skills and current technology learned/enforced:

    In technology, Ithink I dont have any technology that enforced in

    my work in the office building of PHC because the common work there ir

    the technology we use is the computers and Im familiar to that, I think ts

    just reviewed the things that I learned before and it helped me to devel-oped and enhanced it even in the simple and similar task like MSWord, MS

    Excel and etc., those things helped improved my skills that I learned be-

    fore. I learned different acronyms that are realated to Cardiovascular

    operations because I used to encode the different operations.

    B. Equipment, tools, testing apparatus handled (software's used included)

    I don't have any tools, equipment and apparatuses that handled in

    PHC because I wasn't assigned in the hospital building but I was able touse their database called MEDTRAK this is where they input medical rec-

    ords of the patients, sometimes their database have some problems and it

    becomes slower when many of the staffs use it at the same time.

    C. Strong points versus weak points

    My strong points based on my personality is I am willing to do any

    task that they asked me to do, I am willing to learn from them, I am a fast

    learner , adaptable and responsible in whatever the task is assigned tome. My strong points based on my skills is my knowledge on the technolo-

    gy because we commonly use computers in the office work. My weak

    points is Im having a hard time to focus on the task when I started doing it

    and also think that I lack in confidence because Im a very shy person so I

    dont talk too much in the office.

    VII. SELF-ASSESSMENT

    D. Best experiences on the job:

    My best experiences of my training in Philippine Heart Center is when

    they appreciate the things I have done, when I met a new persons, staffs,

    doctors or even patients. Im very happy when they appreciate the effort

    that I do to them every time they ask me to do something. And also whenthe doctors, staffs and I are having a great time that we eat together and

    get to know it each other.

    E. Evidence of Background preparation:

    Faults committed (its causes and suggestion given by the supervisor):

    The first fault that I committed is when I was writing the DTR of a doc-

    tor, I replaced the afternoon time out in afternoon time in, I told my super-visor and I recommend that if we can renew the DTR but my supervisor

    said that we cant because the doctor just signed the DTR. She said that its

    okay and just put an correction fluid. The second fault that I committed is

    when Im going to give a letter to the Cardiovascular Surgeons and doc-

    tors my mistake I when I entered the clinic of Dr. Quitiquit even if theres no

    one inside, actually I didnt expect that theres no staff there because the

    television is on so I thought that there is someone inside, then when I go

    out I met the secretary of the doctor then I apologize for entering the clin-

    ic without the permission and the secretary told me that next time I shouldnot enter a room without seeing a staff inside because she said that if

    something lost in there I could be one of the suspect. My supervisor didn't

    know anything about these incident. The Third fault that I committed is

    when I was ask to give a paper that will be received in HR Department, I

    left the File Copy of the paper then the staff reminded me about that then

    I went back to HRD to get the copy but the person that I left the paper

    with has misplaced the paper then she called the staff and they talked

    about that and she said that if she find the paper she will give it back to

    our department. I apologize and sorry for what I did. I dont know if my su-pervisor know this incident but I didn't told her.
