This inspection was requested by Genetha Rice-Singleton, P.E., State Program Delivery Engineer. The Project Manager is Matt Bennett.

The plans were prepared by Moreland Altobelli Associates, Inc.

The report was prepared by John Kopotic, Design Review Engineer II, and approved by Lisa L. Myers, State Project Review Engineer, Office of Engineering Services. / Victor Dang, Federal Highway Administration.

The NEPA document was not approved prior to the PFPR; however a waiver to conduct the PFPR prior to NEPA approval was granted. The NEPA document was subsequently approved on March 12, 2013.

The Project Manager is reminded that as of the PFPR date of October 4, 2012, NEPA evaluations were still underway and were not completed until March 12, 2013. The Project Manager is advised that the information contained in this report was based on incomplete environmental condition studies and possible commitments as were known at the time. The PFPR inspection team’s review was limited to these incomplete environmental condition studies and possible commitments. Verify that all commitments from the completed approved NEPA document have been incorporated into the plan set and Special Provisions.

This report is being distributed via E-mail. Final plan preparation can now begin.

The Inspection Plans were reviewed in Camden County Annex Building prior to the site inspection.

Projects Let to construction after July 1, 2013 will require the use of the 2011 AASHTO “GREEN BOOK”, GDOT Design Manual, 2011 Roadside Design Guide, 2012 Guide for the Development of Bicycle Facilities and the 2001 Georgia Standard Specifications. Please revise all notes that make reference to previous GDOT Design Manuals and Specifications. Any substandard features that cannot be complied with due to project restraints will require the submission of a design exception/variance to the State Design Policy and Support Engineer.

PROJECT DESCRIPTION

The proposed project will widen and improve the existing two-lane Colerain Road to a four-lane divided roadway with 20-foot raised median and auxiliary lanes at major intersections. The project has a total project length of 5.85 miles. This project has a Management R/W of December 14, 2012.

The Environmental Resources Impact Table (Environmental Resources Impact Table) will be coordinated with the project's Environmental Commitments Table and other plan notes to illustrate the restrictions associated with various environmental resources. The responsibility for inserting the table into the plans will rest with the designer, and its content and accuracy will be confirmed by the NEPA analyst who is responsible for environmentally certifying the project. GENERAL SPECIAL PROVISION COMMENTS:

Contracts Administration should include the Special Provision adding the Fine Grader because this is widening project with at least one mile of continuous full width lanes.

Restrictions to traffic interruptions are recommended.

A. The contractor shall not install lane closures; perform flagging, or move equipment or materials on the travel way that interferes with traffic flow between the hours of 6:00 A.M. to 9:00 A.M. and 2:00 P.M. to 6:00 P.M. Monday through Friday.

B. Interstate lane closures:

Single lane closures:

1. Single lane closures will be allowed between the hours of 9:00 P.M. to 5:00 A.M. Sunday through Saturday.

Double lane closures:

Overall, the Consultant’s electronic files are not at an acceptable level of compliance with the GDOT Electronic Data Guidelines standards. See Appendix “B” for the Compliance Report. Correct the electronic CADD files and ensure they comply with GDOT Electronic Data Guidelines standards.Will correct the naming discrepancies in electronic file and provide subfiles prior to submittal of final plans. 05-XXX TYPICAL SECTIONS

GDOT has defined the Safety Edge as the standard treatment for the outside edge of all uncurbed pavements whether edge of travel lane or shoulder (excluding pavement behind guardrail). If it is not practical to provide the Safety Edge, then a decision to omit shall require a comprehensive study by an engineer and the prior approval of a Design Variance from the GDOT Chief Engineer. The Safety Edge placement should be noted on the construction plan typical sections, with reference to GDOT Construction Detail P-7 for details and method of quantity calculation. Verify that Safety Edge treatment is referenced throughout Typical Sections accordingly.

Will add Const Detail P-7 to index and final plans. Will reference detail on all applicable typical sections

In accordance with the PPG Section 05.002 Required Information, Minimum and maximum dimensions of variable-width features shall be labeled.Will add min and max dimensions to variable widths

Per the GDOT Pavement Design Manual, if paving material is supplied from Florida or soil cement, top soil or other soil bases are used, then show a Square Yard measure for payment if weighing is inconvenient. If Square Yard measurement is specified, the typical section shall be clearly dimensioned along the top finished surface and dimensions noted as the “Width of Payment.”

Will coordinate with others to determine “if weighing is inconvenient” prior to dimensioning and labeling Width of Payment along the top finished surface of roadway typical(s) per GDOT Pavement Design Manual. According to the Design Manual, Appendix I, soil-cement is not to be used in hatched area which encompasses Camden County.

Due to the very granular native soils found in this region, District 5 Construction Office recommends that the graded aggregate base be deleted underneath the proposed curb & gutter. Curb and gutter constructed in this region of Georgia is normally installed directly on the subgrade.Will delete GAB under and behind curb and gutter

Revise the typical sections to clearly define the paving and base courses. Remove the flags “Pavement Section No. 1” and “Pavement Section No. 2”. Revise the Pavement Legend by adding a “callout” for each course. Flag each course with appropriate callout.Will delete GAB under and behind curb and gutter

Include Pavement Reinforcement Fabric for area where you’re widening existing. Add the Pavement Reinforcement Fabric detail.

The District 5 Construction Office recommends showing areas within Typical Section No. 10 and Typical Section No. 11 as Mill and Inlay resurfacing.Will show and label mill and inlay on typical sections and x-sections

Drawing 05-009

Verify the Profile Grade location. It appears it should be located at the right side of the median as shown in the cross sections.The profile grade is shown on the construction centerline of the rural section. The 2’ gutter and 2’ additional pavement width for the turn lane left of the PGL slopes the same as the eastbound thru lanes for sheet flow away from the proposed median. The elevation difference between the proposed median pavement and the existing surrounding ground cannot accommodate an outfall from a median drainage structure which would intercept the runoff prior to crossing Colerain Road as concentrated flow.

Drawing 05-011

The current ramps are in bad condition. Consider upgrading ramps to PCC.In the August 31, 2009 Value Engineering Study-Responses number 13, Value Engineering Recommendation RD-19- Overlay existing Ramps and widen to the inside, it was noted that District 5 representatives commented in the Value Engineering introduction meeting that “There are no visible problems with the existing ramp pavement. Therefore it is recommended that the existing ramps be milled or leveled, overlayed and widened where feasible within the limits of reconstruction.” An e-mail dated September 29, 2009 stated that the “PDC concurs with the District to overlay the existing ramps and reconstruct the interchange to current standards in a future Widening and Interchange Improvement Project.” The September 30, 2009 Value Engineering Implementation Approval Letter recommended implementing the VE Study Alternative RD-19, Overlay existing Ramps and widen to the inside.

The construction plans currently show the southbound off-ramp to be overlayed and widened, the northbound off-ramp to be overlayed and widened with minimum reconstruction to tie to the proposed Colerain Road and the southbound and northbound on-ramps to be reconstructed to tie to the proposed Colerain Road.

The June 17, 2009 correspondence from the GDOT Office of Materials & Research regarding Pavement Type Selection and Pavement Design Recommendation recommended an asphalt pavement section in lieu of concrete for any required full depth reconstruction.

Drawing 05-012

Verify if there we be any work along I-95. Typical Section No. 20 shows no work to be performed on I-95 but Drawing 15-002 & 35-0001 appears to have widening and a TP 7W wall along the face of the columns. Please clarify.Will show and label 4” concrete slope paving at bridge end rolls and median ditch per 15-002. Proposed roadway template on bridge P&E drawing 35-001 will be revised match drawing 15-002 versus ultimate I-95 build out.

The new 31-inch guardrail standards use a 12” offset block instead of the 8” offset block used on the 27” guardrail. Revise the shoulder width dimension to account for the additional 4 inches. If 2’ distance from back of post to shoulder break is not provided a 9’ post is required. Ensure 9’ posts are noted as required for guardrail.Will increase rural shoulder with guardrail width from 15’-6” (10’+2’+3’-6”) to 15’-10” (10’+2’+3’-10”) and urban shoulder with guardrail from 19’-6” to 19’-10” on drawing 5-15 and cross-sections.

There are many drainage structures in the median, it does not appear drainage will enter these boxes. Verify the need for these boxes. The District 5 Construction Office recommends sloping all pavement from the median toward the outside.The profile grade is shown on the construction centerline of the urban section. At locations with normal crown the 2’ gutter and 2’ additional pavement width for the turn lane left of the PGL slopes down towards the proposed median. The elevation difference between the proposed median and outside edge of pavement can accommodate a pipe from the median drainage structure. Sloping said gutter and pavement away from the median would result in exceeding the max 8’ gutter spread therefore requiring additional catch basins along the outside edge of pavement. The proposed pavement cross slope would require warping to match the normal crown through the width of the median opening.

The proposed drainage structures appear to conduct stormwater runoff into the public drainage system. The drainage structures and pipe should be placed in permanent easement at a minimum, preferably in right of way. Driveway easement may also be required to accommodate the tie-in.The proposed relocated pipe and drainage structures along Settlers Hammock Way, a private road, replace in kind the existing system which serves as detention for the site.

The proposed realigned/relocated Brazell Road on the north side of Colerain Road and the associated right of way acquisition appears to be legally defensible due to Brazell Road being an existing public right of way that serves multiple property owners besides just the two gas station sites located in the northeast quadrant of I-95 and Colerain Road. However, the service road proposed for the southeast quadrant does not appear to be legally defensible from a right of way acquisition standpoint as it primarily serves and benefits only one property - Parcel 14. The perceived access impairment on Parcel 14 by limiting access to right-in/right-out is not compensable under Georgia law so there is no damage to "cure". In addition, the law does not allow government to condemn one property to "cure" an impact on another property. In this

This map should not be the same as the drainage area map. This map is required for projects that disturb one acre or more. This map is similar to the drainage area map (Section 21) but is prepared specifically to meet the requirements of the GAR 100002 NPDES Permit. On this map, the drainage areas are delineated with respect to the project stormwater outfalls; while on the map of Section 21, the drainage areas are delineated with respect to the stormwater infrastructure inlets. For each outfall, tabulate a summary (or otherwise summarize) of the hydrologic and hydraulic data specified in the PPG on the map or on a following sheet as drawing space permits. All area calculations shall be in acres.Will revise drawing to show required info outlined in Section 53.002 of the PPG.

The City of St. Marys is maintaining the drainage canal, STA 287+20 +/- LT & RT. Access is not provided for the City to get into this area. Revise the plans to provide a drive on the north & south side of the project for maintenance of canal.Will develop and submit for approval an access plan to the City’s existing r/w for drainage ditch as graphically provided on the City’s existing water and sewer plans received August 7, 2006.
