Tips for Getting StartedAll Vitality Points activities begin online, use this tool to help you navigate each step. You may be asked to register and log in at certain times in the process for your personal identity protection.

1. Access the Village Vitality website through the Benefits link on the VillageWeb or go to http://villagevitality.davita.com (use your DaVita username and password to log in)

2. Select “Vitality Points” in the navigation bar. 3. Click on the “Access Vitality Points Action Plan here” link.

• Be prepared to complete “Security Set-up” and verify your personal information the first time

• Select a Healthy Action to complete from the “Your Action Plan”

Tobacco Free1. > “2011 Tobacco Status” tab on the right side of the screen.

2. > “I’m Tobacco Free Pledge” if you are tobacco-free or “Enroll in CIGNA Quit Today Program” if you plan to quit.

It’s your opportunity to learn important information about your health that is key to prevention and early detection of serious health conditions. It’s also your chance to earn Vitality Points and lower your 2012 DaVita health insurance premiums. Complete the following three healthy actions by November 30, 2011 and save up to $800.

Vitality ScreeningMany people are at risk for serious illness and are not aware of it. That makes early detection important. Did you know:

• One in three adults has high blood pressure—of these, 33% do not get treatment

• One in three adults has high cholesterol—of these, 50% do not get treatment

We are partnering with Onsite Health Diagnostics to give you four confidential ways to get your free screening. Choose to get a screening from one of the following options:

1. Attend an on-site Vitality Screening event at your facility or one nearby2. Visit a participating Lab Corp3. Receive an annual physical from your physician (in-network provider)4. Complete your screening at home with a Home Kit (kit must be returned)

FASTING for an Accurate Total Cholesterol & Glucose Reading

This year Vitality Screenings are fasting only. Fast eight hours before your appointment. Consuming liquids such as coffee and tea are acceptable, but hold the cream and sugar!

CIGNA Health AssessmentThis year you can earn Vitality Points when you complete the CIGNA health assessment (HA). Results are confidential and will provide you with:

• A personal health profile • Strategies and resources for reducing your health risks and maintaining good health

Visit villagevitality.davita.com > Vitality Points tab, and follow the prompts to complete your HA. Teammates participating in Kaiser, Group Health, BCBS and CIGNA plans can earn Vitality Points only when they complete the CIGNA HA.

Be a Wellness ChampionWellness Champions play a big role in helping teammates get involved with activities that improve their Total Wellness. The Village depends on them to:
