Health Science Center Clinical Faculty

Clinical Faculty will be enrolled in HSC college benefit plans such as Health, Life, Accident and Long term disability.

Other State/UF supplemental plans are also available through the main Benefits office at 392-2477.

Important Benefit Reminders

New Hires 60 days from the date of hire to enroll in benefits (Benefit coverage begins on the 1st day of the month AFTER enrollment.)

Annual Enrollment (Benefits Only)Each fall around mid-September to mid-OctoberEnroll/cancel/change with an effective date of January 1 of the following year

Steps to Enroll in Benefits

1. Listen to the Benefits Tutorial (plan information)http://www.hr.ufl.edu/benefits/tutorial/2011/Benefits2011%20FINAL.html

2. Attend a Group Enrollment Session (hands on enrollment assistance)

Sessions to enroll in September coverage are:Friday, 8/19 -- 1:30-3:30 pm Monday, 8/22 -- 9:00 – 11:00 am

Scheduled in Room119 in the HRS Building Sessions after 8/26 are available, however it will result in a

later coverage effective date

To register: sign-on to myUFL. Navigate to My Self Service > Training & Development. Enter course # GET221. Call 392-4626 if you are unable to register

Qualifying Status Change Events

Major “life or work events”

You must make changes within 31 days of the event

Changes must be consistent with the event and may require additional documentation

Important Reminders

Examples of Qualifying Status Change Events

Marriage/Divorce Birth/Adoption (even if you already have family coverage) Change in dependent’s eligibility Change in employment status of employee

or spouse (part-time, leave of absence, etc.) Loss of other insurance coverage

If you’re uncertain whether an event allows a change to your benefits, always contact theBenefits Office for guidance

2012 Plan Year ChangesOnly one HMO per county – Avmed will serve the Gainesville area

If your HMO is no longer offered in your home or work county-- the state will automatically enroll you in the Blue Cross Blue Shield PPO Plan effective 1/1/2012

You must take action during open enrollment if you do not want the PPO plan

Medco will replace Caremark as the Pharmacy Benefits Manager (PBM) and will be the PBM for all HMOs plans
